Instrument-based ocular photoscreening is considered unproven and not medically necessary for children less than 1 year of age. The policy excludes this age group due to insufficient evidence of safety and/or efficacy and documented high unreadable rates reported in published screening cohorts. For example, Longmuir and colleagues reported unreadable rates as high as 25.5% in the 6–11 month group and >20% in infants under 1 year in large programmatic series, supporting the decision to exclude children younger than 1 year from routine instrument-based photoscreening.

For pediatric visual acuity screening the policy endorses specific, validated optotypes and explicitly states that other optotypes such as Allen pictures and the Tumbling E should not be used. Best-practice testing uses appropriately scaled logMAR single crowded HOTV letters or LEA Symbols with crowding bars (typically at a 5-ft/1.5-m test distance) per NCCVH guidance.
