Added goniotomy, trabeculotomy, ab‑interno canaloplasty, and combined canaloplasty/trabeculotomy (e.g., OMNI, Streamline) as proven and medically necessary when performed with cataract surgery for adults with mild–moderate open‑angle glaucoma on ocular hypotensive medication.
Added laser procedures (laser trabeculoplasty, laser iridotomy/iridectomy, laser iridoplasty, and laser ciliary body destruction) to the list of proven and medically necessary indications.
