Coverage Criteria for Obstetrical (Prenatal) Ultrasound

Medically Necessary Ultrasound Coverage

Covered when ALL of the following are met for routine pregnancy care or specific clinical indications:

Routine timing and indications: One ultrasound during the first trimester for indications such as confirmation of intrauterine pregnancy, diagnosis/evaluation of multiple gestation, estimation of gestational age, or confirmation of viability; one ultrasound during the second trimester (generally 18–22 weeks) to survey fetal anatomy and determine gestational age; and one ultrasound during the third trimester for evaluation of placental location, fetal presentation, late registrant evaluation, or assessment of fetal growth.

Up to three obstetrical ultrasounds are recognized during routine pregnancy care; additional imaging may be covered in high-risk pregnancies when results will influence therapeutic decisions.

Types of exams: Standard (basic) examinations include fetal presentation, amniotic fluid volume, cardiac activity, placental position, fetal biometry, fetal number and an anatomic survey; limited examinations address a specific question and do not replace a standard exam; specialized/detailed examinations are used when an anomaly is suspected or in high-risk pregnancies and are performed by experienced operators.

High-risk pregnancies: Additional ultrasounds during a high-risk pregnancy are covered only when the treating provider will make therapeutic determinations based upon the results.

Unproven and Not Medically Necessary Uses

Not medically necessary / unproven uses (explicit):

Unproven uses: Use of prenatal ultrasound solely to determine fetal sex (unless sex determination is essential to diagnosis), three-dimensional (3‑D) prenatal ultrasounds, and more than one detailed fetal anatomic ultrasound examination per pregnancy are considered unproven and not medically necessary.

Detailed fetal anatomic ultrasound coverage

Detailed fetal anatomic ultrasound (CPT 76811) is covered when ALL of the following are met:

Indication-driven coverage for CPT 76811: Known or suspected fetal anatomic abnormality; genetic abnormality; known fetal growth disorder; or increased risk for fetal anatomic or genetic abnormality (examples: prior affected fetus/child, maternal pregestational diabetes, assisted reproduction, multiple gestation, high maternal BMI >= 35 kg/m2, abnormal maternal serum analytes, abnormal screening results, teratogen exposure, isoimmunization, oligohydramnios or polyhydramnios).

Indications list is illustrative, not exhaustive; refer to practice parameters for full listing.

Provider expertise and frequency: The exam should be performed by a practitioner with special expertise and training; no more than one detailed fetal anatomic ultrasound is considered medically necessary per pregnancy per practice unless extenuating circumstances are documented.

If required structures are not visualized during the detailed exam, a focused reassessment (CPT 76816) may be performed.

Coverage and nonmedical use stance

Coverage aligns with established medical indications and discourages nonmedical use:

Medical Indication Required: Ultrasound is covered when performed for a documented medical indication consistent with professional guidelines (e.g., ACOG/AIUM) and when the expected diagnostic benefit justifies fetal exposure.

Not Covered/Discouraged Uses: Ultrasound performed solely to view the fetus, determine fetal sex, or obtain keepsake photos/videos is inappropriate and discouraged by ACOG and the FDA; such nonmedical services are not justified for fetal exposure and may be excluded from coverage.

FDA strongly discourages 3D/4D keepsake imaging; ACOG opposes ultrasound without medical indication.

This policy does not apply to obstetrical ultrasound procedures rendered in the emergency department, during outpatient Observation Care, or in an inpatient hospital setting. These care settings and their associated services are administered under separate inpatient/ED/observation benefit rules and should be billed and reviewed according to those site-specific policies.

Ultrasound examinations performed without a valid documented medical indication (including exams done solely to view the fetus, obtain images/videos, or determine fetal sex for nonmedical reasons) are considered inappropriate and may be denied. Professional statements and practice guidelines recommend that obstetrical ultrasound be performed only when there is a valid clinical reason and when the expected diagnostic benefit justifies fetal exposure.

Use of 2D, 3D, or 4D ultrasound solely to view the fetus, determine fetal sex, or obtain ‘keepsake’ photos/videos is discouraged and considered inappropriate. ACOG notes that although 3D/4D offers technical advantages, there is no proven clinical benefit for routine prenatal diagnosis, and the FDA strongly discourages use of 3D/4D devices to create keepsake images; fetal exposure should be limited to studies with anticipated medical benefit.

Three-dimensional (3-D) prenatal ultrasounds and ultrasound performed solely for fetal sex determination are identified as not medically necessary. Additionally, more than one detailed fetal anatomic ultrasound (CPT 76811) per pregnancy per practice is considered not medically necessary unless extenuating, well-documented clinical circumstances exist. Keepsake/entertainment imaging lacking a medical indication is likewise excluded from coverage.

Routine use of Doppler ultrasound in unselected, low-risk pregnancies and routine late-pregnancy ultrasound after 24 weeks in low-risk or unselected populations have not demonstrated clear clinical benefit and are not supported as routine coverage. Doppler and late-pregnancy ultrasound remain appropriate when indicated by maternal or fetal conditions that would change management.

Ultrasound examinations performed without a documented medical indication or anticipated medical benefit (commonly described as keepsake or entertainment scans) are not medically necessary and may be denied. Professional societies and regulatory guidance specifically caution against nonmedical ultrasound exposure and recommend adherence to ALARA principles.

Detailed fetal anatomic ultrasound (CPT 76811) is indication-driven and reserved for known or suspected fetal anatomic abnormality, genetic abnormality, known fetal growth disorder, placenta accreta spectrum, or other high-risk indicators. No more than one detailed fetal anatomic ultrasound is medically necessary per pregnancy per practice unless extenuating circumstances exist. If required structures are not visualized on the detailed exam, document limitations and consider a focused assessment (CPT 76816) as appropriate.
