Inpatient maternity care with minimum stays: Inpatient hospital care for maternity is covered, including labor, delivery, recovery, physician services, and ancillary services. A minimum 48-hour inpatient stay after a normal vaginal delivery and a minimum 96-hour inpatient stay after cesarean delivery are covered; earlier discharge is allowed if the treating physician and mother agree. If discharged earlier than the minimum, a post-discharge follow-up visit for mother and newborn will be provided within 48 hours when prescribed by the treating physician. Services of a licensed/Certified Nurse-Midwife are covered when available within the member's network/participating medical group. Circumcision coverage follows specified timing rules.

Health insurance is not required to cover expenses arising from abortion except in limited circumstances. Consistent with the Pregnancy Discrimination Act and related guidance, coverage for pregnancy-related conditions must be provided on the same terms as other medical conditions; however, coverage for abortion-related services is not mandated except where the life of the mother would be endangered or when medical complications arise from an abortion. Plans must apply the same limits, deductibles, and reimbursement rules to pregnancy-related claims as they do to other medical conditions.

The policy explicitly excludes certain non-medically indicated diagnostic tests and elective services. Examples include procedures performed solely for sex determination (e.g., ultrasound for that purpose), blood testing to determine paternity, elective home delivery (unless separately covered under mandated regulations), childbirth classes and other educational courses on childcare, and take-home medications or supplies when not covered by a supplemental pharmacy benefit. Also excluded are newborn coverage of a grandchild except in narrow circumstances and maternity services provided for a non-member acting as a surrogate.

A recent revision added an explicit Not Covered item for genetic screening of newborns when it is not immediately beneficial. Specifically, the policy now excludes non-medically necessary screening of newborns to determine carrier status for inheritance disorders when there would not be an immediate medical benefit or when results would not be used to begin medical interventions/treatment while a newborn. This change is recorded in the policy history effective 05/01/2025.

Provider Requirements, Prior Authorization, and Documentation

Direct access to OB-GYN (CA HSC 1367.695)

California enrollees have direct access to participating obstetrician-gynecologists (and participating family practice physicians/surgeons designated to provide OB-GYN services) without prior approval from another provider or the plan, per CA HSC §1367.695. Plans may set reasonable, non‑restrictive utilization requirements and may require participating OB‑GYNs to communicate with the enrollee's primary care physician regarding care and follow‑up.

Do not require prior authorization or referral for enrollee to see a participating OB‑GYN for obstetrical or gynecological services.
