Not medically necessary / investigational when used for breast cancer screening, diagnosis, or to guide surgery:

Primary exclusion list: Breast ductal lavage; Breast ductal fluid aspiration and cytology; Fiberoptic ductoscopy, with or without ductal lavage are considered unproven and not medically necessary for screening, diagnosis, or as alternative tools to guide surgery due to insufficient evidence of efficacy.

Supported by evidence demonstrating limited sensitivity, high inadequate sample rates for NAF cytology, and lack of guideline support (NCCN); these procedures are therefore not recommended for routine screening, diagnosis, or to guide surgery.

Use and limitations of FDS and NAF cytology

Evidence-based considerations for use of FDS/NAF cytology in PND and cancer detection:

Imaging-first and selective FDS: Prefer high-frequency ultrasound (HFUS) for evaluation of pathologic nipple discharge (PND); perform fiberoptic ductoscopy (FDS) when HFUS cannot establish the cause, location, or extent of the lesion or is non-diagnostic.clinical judgment

Retrospective data indicate HFUS is noninvasive and preferred for localization; reserve FDS for cases where HFUS fails to reveal lesions.

The policy considers the following procedures unproven and not medically necessary for use in breast cancer screening, breast cancer diagnosis, or as alternative tools to guide surgery due to insufficient evidence of efficacy: breast ductal lavage; breast ductal fluid aspiration and cytology; and fiberoptic ductoscopy, with or without ductal lavage. These services are listed together because current evidence does not demonstrate that they improve diagnostic outcomes or are an appropriate substitute for established screening and diagnostic modalities.

Available studies of fiberoptic ductoscopy (FDS) are primarily preliminary cross‑sectional investigations and small trials assessing technical success and diagnostic accuracy rather than patient‑level outcomes. Evidence limitations include technically challenging procedures with restricted visualization of the ductal system, small or heterogeneous study populations, variable definitions of a positive ductoscopic finding, and tradeoffs between sensitivity and specificity depending on positivity criteria. Importantly, there is no high‑quality evidence demonstrating that ductoscopy improves breast cancer detection rates, leads to earlier stage at diagnosis, or reduces breast cancer mortality; further outcome‑oriented studies are needed to substantiate any clinical benefit.

Coverage determinations under this Medical Policy must be applied in the context of the member’s specific benefit plan. When there is a conflict between the terms of this Medical Policy and the member specific benefit plan document, the member specific benefit plan governs. Providers and adjudicators should verify applicable plan provisions and any federal or state mandates before making a final coverage decision.

Based on guideline recommendations and the evidence summarized in this policy, ductal lavage and nipple smear cytology are not recommended for routine screening or for evaluation of patients without palpable symptoms. These procedures are considered investigational or not medically necessary for asymptomatic screening because of high inadequate sample rates, limited sensitivity, and lack of demonstrated improvement in clinically meaningful outcomes.

Nipple aspirate fluid (NAF) tests and other nipple aspirate procedures should not be used as substitutes for mammography or standard breast cancer screening programs. The U.S. Food and Drug Administration has noted that nipple aspirate tests are not substitutes for mammograms, and inclusion of NAF‑based assays in this policy does not indicate they replace established screening modalities.

Background

Cytologic analysis of epithelial cells collected from breast ductal fluid or nipple aspirate fluid has been investigated as a potential early indicator of breast cancer using techniques such as ductal lavage, nipple aspiration, and fiberoptic ductoscopy. These approaches have been studied for screening, diagnostic evaluation (including pathological nipple discharge), and as adjuncts to image‑guided assessment. However, studies report frequent inadequate samples, variable sensitivity and specificity, and limited ability to visualize peripheral ductal lesions, which together have limited the clinical adoption of these methods. As a result, cytologic evaluation of ductal or nipple fluids remains investigational for routine screening and unproven as a replacement for established diagnostic pathways.
