Use of cytology/NAF and ductoscopy is supported under limited, specific circumstances and with recognized limitations:

Preferred initial evaluation for pathologic nipple discharge (PND): High-frequency ultrasound (HFUS) is the preferred, noninvasive initial evaluation for pathologic nipple discharge; fiberoptic ductoscopy (FDS) is appropriate only when HFUS cannot establish the cause, location, or extent of the lesion.

Yuan et al. and retrospective studies concluded HFUS preferred; FDS reserved when HFUS inadequate.

Diagnostic yield and evidence context: Ductoscopy can detect many underlying malignancies (systematic review pooled sensitivity for 'any' ductoscopic findings ~94%) but has limited specificity and substantial heterogeneity across studies; cytology of NAF/ductal fluid has low sensitivity and high rates of inadequate samples and therefore offers limited standalone diagnostic value but may be complementary in select settings.

For pathologic nipple discharge (PND), high-frequency breast ultrasound (HFUS) is generally the preferred first-line evaluation. Fiberoptic ductoscopy (FDS) may be considered only when HFUS and other conventional imaging cannot localize or characterize the lesion, and after documenting the rationale and prior imaging results. Ductal lavage, nipple aspirate fluid cytology, and similar tests are considered unproven and not medically necessary for screening, routine diagnosis, or as an alternative to surgery.

When ordering FDS or DL, document why HFUS and conventional imaging were insufficient.
If submitting CPT 19499 for an unlisted ductal procedure, include detailed operative report and justification.
