Background & Evidence

Cytologic examination of epithelial cells from breast ductal fluids (obtained via ductal lavage, nipple aspiration/NAF, or fiberoptic ductoscopy) has been studied as a potential early indicator of breast cancer. Studies demonstrate key shortcomings: low sensitivity in detecting malignancy in many series, issues with sample adequacy including high proportions of inadequate samples, and high rates of inconclusive results. There is no evidence that adding these methods to standard screening reduces breast cancer mortality, and ductoscopy is technically challenging with limited ductal access and uncertain impact on outcomes.
