This policy governs coverage criteria for Viltepso (viltolarsen) for treatment of Duchenne muscular dystrophy (DMD) amenable to exon 53 skipping for UnitedHealthcare Community Plan members in NC.
Coverage criteria for initial therapy were revised to require either no prior gene therapy for DMD or, if prior gene therapy was received, documentation of clinically meaningful functional decline since gene replacement therapy.
Revised coverage criteria for initial therapy to require one of the following: the patient has not previously received gene therapy for DMD, or both that the patient previously received gene therapy and submission of medical records documenting a clinically meaningful functional decline since receiving gene replacement therapy.
