Inpatient vEEG — Medically Necessary Conditions

Inpatient vEEG monitoring is considered medically necessary when InterQual criteria are met or when any ONE of the following circumstances applies:

Inpatient vEEG necessity conditions

Individual is not expected to have a seizure or seizure-like diagnostic event within a timeframe that is reasonable for an ambulatory vEEG recording.
Individual is undergoing preoperative evaluation for epilepsy surgery.
Seizure provocation maneuvers are required that warrant direct observation in an inpatient setting.
Seizure medication is being adjusted in such a way as to risk provoking an event that would require inpatient management.
Seizure medication discontinuation is required to provoke seizure for diagnostic purposes.

Clinical Indications (professional society recommendations)

vEEG/LTM is indicated (covered) in outpatient or inpatient settings when used for ALL of the following clinical goals as supported by professional society guidance:

vEEG clinical indications: Detection, characterization, and quantification of ictal events to confirm epilepsy versus non-epileptic events; differential diagnosis between epileptic and non-epileptic conditions (including psychogenic nonepileptic events and sleep-related paroxysmal disorders); classification of seizure type or epilepsy syndrome when unclear; documentation of electroclinical manifestations of habitual seizures for presurgical evaluation; documentation of diurnal/circadian patterns and response to pharmacological interventions; evaluation of seizure patterns during sleep and related sleep-architecture effects (including pediatric cognitive epilepsy contexts); and monitoring in the ICU for identification of subclinical seizures/status and to assess effectiveness of treatment for status epilepticus.

Prior Authorization

Inpatient Necessity Triggers

Prior authorization is required for inpatient vEEG admissions when InterQual® criteria apply. Refer to the InterQual® criteria for video EEG monitoring. If InterQual® criteria are met, inpatient admission is proven and medically necessary for any of the following: the individual is not expected to have a seizure or seizure-like diagnostic event within a timeframe reasonable for ambulatory vEEG; the individual is undergoing preoperative evaluation for epilepsy surgery; seizure provocation maneuvers are required that warrant direct inpatient observation; seizure medication is being adjusted in a way that risks provoking an event that would require inpatient management; or seizure medication discontinuation is required to provoke a seizure for diagnostic purposes.

Ambulatory vs Inpatient Considerations

Consider ambulatory/outpatient vEEG monitoring when diagnostic events are likely to occur within a reasonable ambulatory monitoring window and when the risk of provoking or managing a seizure inpatient is low. Ambulatory monitoring may be appropriate when event frequency and timing suggest capture is probable without medication withdrawal or other inpatient-only provocation maneuvers. Choose inpatient monitoring when events are unlikely to occur during outpatient monitoring, when medication adjustment or withdrawal is needed to provoke events, when provocation maneuvers requiring direct observation are planned, or when preoperative surgical evaluation is required.

Required Clinical Documentation

When requesting authorization or documenting vEEG monitoring, provide clear clinical justification for the chosen setting. Required documentation should include: indication for monitoring, suspected diagnosis or differential (epileptic vs non-epileptic), expected event frequency and timing, prior outpatient/inpatient EEG results and other relevant testing, whether medication adjustment or withdrawal is planned (and rationale), planned provocation maneuvers if any, and whether the individual is being evaluated for epilepsy surgery.

Background and Evidence

Video electroencephalographic monitoring (vEEG) combines standard electroencephalographic (EEG) recording with synchronized video to document clinical behavior at the time of electrographic events. vEEG is used to correlate observed clinical events with EEG activity to distinguish epileptic from non‑epileptic events, classify seizure type or epilepsy syndrome when unclear, document habitual seizures for presurgical evaluation, and assess responses to medication changes or provocation maneuvers.

Long‑term monitoring (LTM) may be performed in either outpatient or inpatient settings and can extend continuously over multiple days. Inpatient vEEG monitoring is particularly useful when medication withdrawal, provocation maneuvers, or other interventions that may provoke seizures are required and when events are unlikely to be captured reliably with ambulatory monitoring.
