Covered when ALL of the following are met:

Primary diagnosis: Recipient has a diagnosis of medically intractable epilepsy with partial epilepsy confirmed and classified according to the International League Against Epilepsy (ILAE).

Recipient may also have associated generalized seizures; VNS may have efficacy in primary generalized epilepsy.

Age and body mass: Recipient is age 12 years or older (case-by-case consideration may be given to younger children who meet all other criteria and have sufficient body mass to support the implanted system).>= 12 years

Medication-refractory: Seizures are refractory to medical anti-epilepsy treatment with adequately documented trials of appropriate standard and newer antiepileptic drugs or documentation of inability to tolerate these medications.

Surgical candidacy: Recipient has undergone surgical evaluation and is considered not to be an optimal candidate for epilepsy surgery.

Seizure frequency and duration: Recipient is experiencing at least 4–6 identifiable partial-onset seizures each month and has had intractable epilepsy for at least 2 years (the two-year requirement may be waived if waiting would be seriously harmful).>=4-6/month; >=2 years
