Small studies, limited sample sizes, and weak designs preclude considering tVNS proven for these indications outside of well‑controlled research.

Use of external trigeminal (supraorbital) nerve stimulation for ADHD is not recommended by the American Academy of Pediatrics because supporting evidence is sparse compared with established behavioral and medication treatments. Separately, NICE restricts routine use of supraorbital stimulation for migraine to acute treatment only with special arrangements for clinical governance, consent, and audit or research, and advises preventive use only in research contexts.

For implantable VNS considered for epilepsy, document shared decision‑making that the patient is not a candidate for resective epilepsy surgery, has failed surgery, or has refused surgery after a shared decision‑making discussion. Include counseling on risks, benefits, and alternatives.
