inv-01: Implantable VNS — Medically Necessary

Covered when ALL of the following are met:

Implantable VNS for epilepsy coverage criteria: Medically refractory epileptic seizures with failure of two or more trials of single or combination antiepileptic drug therapy or intolerable side effects of antiepileptic drug therapy; and the individual is not a candidate for epilepsy surgery, has failed epilepsy surgery, or refuses epilepsy surgery after a Shared Decision Making discussion; and no history of left or bilateral cervical vagotomy (contraindication per FDA).

inv-02: Unproven / Not Medically Necessary Uses

Unproven indications for implantable VNS and non-implantable VNS: Implantable vagus nerve stimulators are unproven and not medically necessary for conditions including, but not limited to, Alzheimer's disease; anxiety disorder; autism spectrum disorder; autoimmune disorders; back and neck pain; bipolar disorder; bulimia; cerebral palsy; chronic pain syndrome; cluster headaches; depression; fibromyalgia; heart failure; migraines; morbid obesity; musculoskeletal disorders; narcolepsy; obsessive-compulsive disorder; paralysis agitans; sleep disorders; Tourette's syndrome; upper limb impairment related to stroke; and other conditions listed in the policy. Transcutaneous (non-implantable) vagus nerve stimulation devices are unproven and not medically necessary for preventing or treating all indications.

Cluster headache: Adjunctive prophylactic nVNS (PREVA) showed a mean therapeutic gain (≈3.9 fewer attacks/week) and higher ≥50% response rates versus standard of care but was open-label; acute nVNS RCTs (ACT1/ACT2, Silberstein) showed benefit for episodic cluster headache but not chronic cluster headache.

Limitations include open‑label design, sham/blinding challenges, and need for long‑term data.

Migraine - prevention and acute: nVNS trials (PREMIUM and others) show mixed results: no superiority in ITT preventive analyses often due to sham device activity and adherence issues; post hoc/adherent-subgroup analyses sometimes show benefit. Acute migraine/cluster headache evidence: Pivotal trials for non-invasive neuromodulation (nVNS, sTMS, eTNS) show heterogeneous results: sTMS demonstrated a significant 2-hour pain-free response vs sham; nVNS and eTNS pivotal acute trials did not consistently reach significance across primary endpoints, though some trials (e.g., Chou et al. 2019) report significant 1‑hour pain reduction with e-TNS.

Heterogeneity among pivotal studies complicates direct comparisons.

ADHD evidence: A blinded sham-controlled trial (McGough et al. 2019) in children 8–12 years showed statistically significant improvement in ADHD‑RS and CGI favoring active TNS, but only slightly more than half achieved clinically meaningful improvement; AAP guidance does not recommend eTNS for ADHD pending more evidence.

Durability and developmental impact require further study (AAP statement).

Depression evidence: Small randomized sham-controlled TNS trials (e.g., Generoso et al. 2019) report short-term symptom reductions, and VNS registry/observational data suggest longer-term benefit for TRD, but randomized evidence is mixed and limited by small samples or design constraints.

Transcutaneous (non-implantable) vagus nerve stimulation devices are considered unproven and not medically necessary for preventing or treating any indication due to insufficient evidence of efficacy. This includes the class of external or transcutaneous trigeminal nerve stimulation devices when used for treating or preventing conditions such as ADHD, depression, epilepsy, and headache, among others; these devices require higher-quality studies before routine coverage can be supported.

Implantable vagus nerve stimulation is not medically necessary/unproven for a range of non‑epilepsy and non‑established indications because available studies are limited, small, or flawed. Specific conditions listed in the evidence include, but are not limited to: Alzheimer's disease, anxiety disorder, autism spectrum disorder, bipolar disorder, cluster headaches, depression, fibromyalgia, heart failure, migraines, morbid obesity, obsessive‑compulsive disorder, sleep disorders, and upper limb impairment related to stroke. For these conditions the policy concludes there is insufficient data to establish safety and effectiveness.

The evidence is insufficient to support noninvasive/transcutaneous VNS (nVNS/tVNS) for migraine; study limitations and small sample sizes prevent definitive conclusions about safety and effectiveness for acute or preventive migraine therapy. Transcutaneous VNS has also been investigated for multiple other indications (e.g., atrial fibrillation, epilepsy, depression, heart failure, pain, tinnitus, sleep quality), but current data are limited and do not establish clinical benefit for those uses.

National Institute for Health and Care Excellence (NICE) guidance (2022) concludes that transcutaneous electrical stimulation of the supraorbital nerve has adequate safety evidence but limited or inadequate evidence for efficacy in preventing migraine. NICE recommends that use for acute migraine should occur only with special arrangements for clinical governance, consent, audit or research, and that use for migraine prevention should be limited to research settings.

Coverage decisions under this policy must default to the member specific benefit plan document and any applicable federal or state mandates. In the event of a conflict between this Medical Policy and the member's benefit plan, the member specific benefit plan governs; providers should verify plan terms and prior authorization requirements before proceeding.

Implantable vagus nerve stimulators are considered unproven and not medically necessary for the enumerated non‑established conditions (for example, Alzheimer’s disease, migraines, depression, cluster headache, obesity, and others listed in the policy) because the available evidence does not demonstrate consistent benefit across these indications.

For depression, the evidence is limited: study limitations and mixed results mean VNS cannot be considered proven outside of research contexts. Similarly, transcutaneous (non‑implantable) VNS is not supported for indications where evidence is inadequate (including some cluster headache subtypes and other investigational disorders); larger, higher‑quality trials are necessary to establish safety and efficacy.

Because of study limitations, small sample sizes, and low or very‑low quality evidence across trials, current data do not support routine use of noninvasive VNS (nVNS/tVNS) or external trigeminal nerve stimulation (eTNS) for many indications. Systematic reviews report some positive signals for migraine and other conditions, but overall the evidence base is insufficiently robust to conclude effectiveness for routine clinical coverage.

The American Academy of Pediatrics reviewed the available trial data for external trigeminal nerve stimulation and concluded that eTNS cannot be recommended for treatment of ADHD in children and adolescents because supporting evidence is sparse and does not approach the strength of established medication and behavioral treatments.
