Coverage Criteria and Medical Necessity

Proven indications for NCS

Covered when ANY of the following known or suspected disorders are being evaluated and when performed with or without standard late responses; neuromuscular junction testing must be performed in conjunction with needle EMG.

Proven indications: Peripheral neuropathy/polyneuropathy (e.g., inherited, metabolic, traumatic, entrapment syndromes); Plexopathy; Neuromuscular junction disorders (e.g., myasthenia gravis); Myopathy; Motor neuron disease; Radiculopathy (cervical, thoracic, or lumbosacral); Treatment guidance (e.g., muscle localization for botulinum toxin injections when required).

NCS may be performed with or without standard late responses (e.g., F‑wave, H‑reflex); neuromuscular junction testing requires concurrent needle EMG as noted.

NCS without needle EMG — permitted scenarios

Nerve conduction studies performed without needle electromyography are medically necessary for individuals with any of the proven indications above when ANY of the following clinical circumstances apply:

Permitted NCS without needle EMG: Individuals treated with anticoagulants; Individuals with lymphedema; Individuals being evaluated for carpal tunnel syndrome.

These are limited scenarios in which NCS alone is acceptable; otherwise NCS should generally be accompanied by needle EMG per professional guidance.

Indications for EDX testing (per AANEM)

Covered when ALL of the following are met (per AANEM model policy and Electrodiagnosis guidance):

EDX testing indicated: EDX testing (nerve conduction studies and needle electromyography) is indicated for focal neuropathies/entrapment (e.g., carpal tunnel, ulnar neuropathy), traumatic nerve lesions, generalized neuropathies (diabetic, uremic, metabolic, toxic, hereditary, immune‑mediated), neuromuscular junction disorders, radiculopathy, plexopathy, myopathy, precise muscle localization for injections, and symptom‑based presentations (pain, weakness, sensory disturbance) when pre‑test evaluations are inconclusive.

Based on AANEM Model Policy and Electrodiagnosis for Distal Symmetric Polyneuropathy guidance

When to seriously consider EDX for distal symmetric polyneuropathy: When history/physical and standard neuropathy labs do not indicate a likely etiology; symptoms/physical findings are moderate to severe; atypical presentation (predominant motor, proximal deficits, or asymmetry); rapid progression; signs suggesting another disorder (e.g., radiculopathy); unknown duration/severity; family history suggesting hereditary neuropathy; exposure to neurotoxic agents; discrepancy between signs and symptoms.

The following tests and technologies are considered unproven or not medically necessary because current evidence does not support their clinical utility:

Not medically necessary tests/technologies: Nerve conduction studies for conditions not listed as proven; Non‑invasive automatic, portable, or automated point‑of‑care nerve conduction systems that test only distal motor latencies and conduction velocities for electrodiagnostic purposes; Macroelectromyography (macro‑EMG); Surface electromyography (SEMG) and SEMG‑based seizure monitoring systems; Surface mechanomyography (sMMG); Physiologic monitoring of seizure or movement disorder symptoms using wearable accelerometer/gyroscope/electrodermal sensor devices (e.g., wrist devices, smartwatches) when intended for diagnostic EDX substitution; Quantitative sensory testing (including monofilament, pressure‑specified, computer‑assisted exams, and current perception threshold testing); Visual evoked potential testing for diagnosis/evaluation of glaucoma.

These items are explicitly listed as unproven and not medically necessary in the absence of stronger evidence.

Wearable movement and seizure devices: Wearable movement and seizure monitoring devices show promise (e.g., PKG, KinetiGraph, smartphone sensors) with studies reporting variable sensitivity/specificity and some impact on management (PKG altered management in a subset), but larger controlled outcome studies demonstrating benefit are lacking.

ILAE‑IFCN conditionally recommends validated wearables for GTCS/FBTCS detection in safety‑sensitive situations; other applications remain investigational.

Coverage considerations derived from evidence reviews and professional society positions:

QST and device validity: QST and related sensory threshold tests should be used only with instruments and methodologies that have demonstrated reproducibility and validated normal values; differences among devices and protocols affect interpretation.

Different commercially available QST instruments have varying specifications and only reproducible methodologies should be used for patient care and research (AANEM/AAN).

sNCT/current perception threshold: Current perception threshold/sensory nerve conduction threshold testing is considered investigational per AANEM model policy.

Devices or techniques lacking validated reproducibility are not supported for routine clinical use.

sEMG clinical utility: Surface EMG has limited and inconsistent evidence across applications (seizure detection adjunct, respiratory monitoring, ALS, SCI, TMD); although some FDA‑cleared devices exist (e.g., SPEAC/Brain Sentinel), overall clinical utility is not established and further well‑designed studies are needed.

AANEM practice topic finds data insufficient to determine sEMG clinical utility for many neuromuscular diagnoses.

This policy does not address intraoperative neurophysiologic testing. Intraoperative monitoring and related techniques are outside the scope of this document and should be evaluated under separate institutional or payer policies.

The evidence for macro-electromyography (macro-EMG) and automated point-of-care nerve conduction tests is limited. Peer-reviewed studies are few and small, and systematic review found overall weak evidence for these modalities; current data do not support routine substitution of these tests for standard electrodiagnostic studies.

Quantitative Sensory Testing (QST) can document sensory abnormalities in some settings but has methodological limitations. Professional guidance notes that QST results should not be used as the sole diagnostic method for neurologic pathology; reproducibility, instrument variability, and dependence on patient cooperation limit routine adoption for many indications.

Current perception threshold / sensory nerve conduction threshold testing (sNCT) is identified by AANEM as investigational. Devices and techniques for current perception threshold testing lack validated, reproducible methodology sufficient to support routine clinical use.

Policy change: the prior list of example devices for non‑invasive, portable, or automated point‑of‑care nerve conduction monitoring systems (e.g., NC‑stat® System, Brevio® NCS‑Monitor, Advance™ System) has been removed from the policy history and coverage rationale.

Nerve conduction studies for conditions not listed as proven, and specified noninvasive/automated systems and technologies (including macro‑EMG, automated point‑of‑care NCS that only report distal latencies/velocities, SEMG/sMMG, QST variants, and VEP for glaucoma) are considered not medically necessary or investigational due to insufficient evidence of consistent, patient‑relevant benefit.

Clinical practice note: performing NCS alone without concomitant needle EMG should be the exception. The AANEM emphasizes that NCS without needle EMG (except in limited, specified circumstances) may omit essential diagnostic information and therefore is generally outside standard electrodiagnostic practice.

Evidence note: the literature supporting surface EMG (SEMG) and SEMG‑based seizure monitoring systems is weak. Available studies do not consistently demonstrate robust, patient‑relevant outcomes, and additional well‑designed research is required to establish clinical utility.

Documentation Required

Supervision and Performer Requirements

Electrodiagnostic testing must be performed or supervised by appropriately trained personnel. The EDX physician should supervise data collection, may delegate test performance to trained technologists or trainees, and should be immediately available for NCS/SEP procedures. Needle EMG must be performed by a physician skilled in EDX medicine.

EDX testing should be performed/supervised by a physician with demonstrable EDX training (neurology or PMR residency/fellowship, ABEM or equivalent).
A technologist may perform NCS/SEP under direct supervision; the supervising physician must review results while the patient remains available and be alerted to unexpected findings.
One physician should supervise and perform all components when indicated; separate reports for NCS and EMG are generally inappropriate.
