Coverage Criteria

Outpatient multiplex panel (six or more targets)

Not medically necessary in outpatient setting when the following condition is met:

Outpatient large-panel restriction: Respiratory pathogen panel testing that includes six or more targets performed in an outpatient settingpanel size >= 6 targets

Guidelines (ASM, ATS, IDSA) identify these populations as appropriate candidates for multiplex or comprehensive testing; use clinical judgment

Respiratory pathogen panel testing that includes six or more targets when performed in an outpatient setting is considered unproven and not medically necessary due to insufficient evidence of clinical efficacy. The larger fixed multiplex panels include pathogens for which simultaneous testing is rarely clinically indicated, and results can be difficult to interpret because of prolonged viral shedding, possible coinfections, and variable accuracy across targets. Such outpatient testing is therefore not covered under this policy unless otherwise required by the member’s benefit plan.
