In the outpatient setting, respiratory pathogen panel testing of six or more targets is considered unproven and not medically necessary due to insufficient evidence of efficacy for outpatient indications. Evidence for broader or advanced approaches such as metagenomic next-generation sequencing (mNGS) and some multiplex panels may be promising in other clinical contexts (for example, hospitalized or immunocompromised patients) but those settings are outside the outpatient coverage rationale described here.

For adult outpatients with suspected community-acquired respiratory infection, guidance and available evidence caution against broad routine nucleic acid testing for noninfluenza viral pathogens. The American Thoracic Society issues a conditional recommendation to avoid routine outpatient nucleic acid–based testing for noninfluenza viruses in adults with suspected community-acquired pneumonia, citing very low–quality evidence. Observational data also suggest that identification of influenza (but not necessarily other noninfluenza viruses) is associated with changes in antiviral and antibiotic prescribing, calling into question the incremental benefit of multiplex testing versus targeted influenza testing in typical outpatient practice.

When performed in the outpatient setting, multiplex respiratory pathogen panels that include six or more targets lack sufficient evidence of clinical benefit and are therefore regarded as unproven. As a result, such large panels in outpatient use are considered not medically necessary and are excluded from routine outpatient coverage.

Multiplex panel testing for adult outpatients who do not have severe illness or high-risk features is discouraged. Clinical guidance and available studies indicate that routine multiplex testing for noninfluenza viruses in otherwise stable adult outpatients has not demonstrated clear additional benefit over targeted testing for influenza (and other priority pathogens) and should generally be avoided unless the patient has severe disease or risk factors that would alter management.
