Covered when ALL of the following criteria are met:

ANY of the following

Testing is performed in an inpatient setting (including ICU) or other high-acuity setting where results will directly inform immediate clinical management.
Examples: ICU patients, hospitalized patients with severe disease.
Patient is immunocompromised or has a condition listed as appropriate for multiplex testing by specialty society guidance (e.g., hematology/oncology patients, transplant recipients, pediatric patients with significant comorbidity).
Per ASM, ATS, and IDSA guidance; see policy rationale.
Testing is performed to guide antimicrobial or antiviral therapy or other urgent management decisions where a comprehensive NAAT panel will change immediate clinical care.
Operational: documentation in the medical record must support how results will affect management.

ALL of the following

Test is ordered for an outpatient setting (including urgent care and ambulatory clinics)
Outpatient includes any non-hospitalized patient encounter.
The ordered respiratory pathogen panel contains six or more targets≥ 6 targets
Fixed large multiplex panels that include multiple unrelated pathogens.
There is no documentation that the patient is at high risk for complications or that results will directly change immediate management (see appropriate-use criteria above).
Routine testing of otherwise healthy adult outpatients is not supported by evidence.

Hospitalized with severe CAP: Hospitalized patients with severe community-acquired pneumonia (CAP) — consider multiplex NAAT testing for non-influenza respiratory pathogens as part of diagnostic evaluation.

ATS suggests considering NAA testing for hospitalized patients with severe CAP; IDSA supports comprehensive panels in hospitalized/ICU patients.

ICU patients: Intensive care unit (ICU) patients — comprehensive respiratory panel testing may be appropriate to guide antimicrobial therapy and management.

Acute respiratory infections are common and most are caused by viral pathogens; multiplex nucleic acid amplification tests (NAATs/PCR) rapidly detect multiple viral and bacterial targets from a single respiratory specimen. While multiplex panels can provide comprehensive pathogen identification, large fixed panels may include agents of variable clinical relevance, detect prolonged shedding or colonization, and produce multiple positives that complicate interpretation. These factors, combined with limited and low‑quality outpatient evidence, underpin the cautious approach to routine use of broad multiplex panels in ambulatory settings.
