Medically Necessary Coverage Criteria

Covered when ALL of the following sets apply (two distinct covered groups described in policy):

Group A — up to 5-target panel coverage

Clinical indications (Group A): Diarrhea for more than seven days; OR diarrhea with at least one of: fever; bloody or mucoid stools; severe abdominal cramping or tenderness; signs of sepsis; OR suspected enteric fever in an individual with recent travel to an endemic region or exposure via recent food preparation by someone with endemic exposure.

Group B — 6-11 target panel coverage for persistent diarrhea in at-risk individuals

At-risk conditions (Group B): At risk for C. difficile colitis and one of: diarrhea for more than seven days; OR diarrhea with at least one of: fever; bloody or mucoid stools; severe abdominal cramping or tenderness; signs of sepsis; OR acquired immune deficiency syndrome (AIDS); OR on immunosuppressive medications (post-transplant or for autoimmune disease); OR other conditions causing immunosuppression and other stool diagnostic studies have failed to yield a pathogenic organism.

Clinical indications: Patient presents with moderate-to-severe, bloody, febrile, dysenteric, nosocomial, persistent (>7 days), or immunocompromised-associated diarrhea.

Test selection rationale: When available, use culture-independent multiplex molecular tests for bacterial pathogen detection due to greater sensitivity and faster turnaround; for C. difficile use a two-step algorithm (sensitive screen then specific confirmatory test) to distinguish colonization from infection.

Considered low yield (coverage discretionary) when ALL of the following apply:

Low-yield testing: Acute, noninflammatory diarrhea of limited duration in otherwise healthy individuals (afebrile, nonbloody, short duration) where symptoms are self-limited and there is no clinical feature indicating testing.

Primary clinical indications: Testing is covered when the patient has moderate-to-severe, bloody, febrile, dysenteric, nosocomial, or persistent diarrhea, or is immunocompromised.

Supports bacterial pathogen testing by multiplex NAATs when available.

Specific scenarios warranting stool panel testing: Include: fever or bloody diarrhea; diarrheal illness with severe abdominal cramping, tenderness, or signs of sepsis; travel with risk for enteric fever when febrile; hospitalization >3 days; persistent diarrhea >7 days (consider parasite testing); recent antimicrobial exposure when C. difficile is suspected; immunocompromised hosts requiring expanded testing.

Also consider testing for Yersinia, Vibrio, parasites, and expanded testing in immunocompromised hosts as noted by IDSA.

Specimen/interpretation cautions: Multiplex NAATs detect nucleic acid and may detect viable and nonviable organisms; they should not be used as tests of cure and multiple-pathogen results require clinical interpretation.

Acute gastroenteritis and noninflammatory diarrhea are usually self-limited and, in otherwise healthy individuals, do not require routine laboratory testing. Clinical features such as short duration, absence of fever, and nonbloody stools predict a low pretest probability of a pathogen that would change management; therefore, testing in these situations is generally low yield. When available and clinically indicated for more severe presentations, culture-independent molecular methods may be used because they offer faster turnaround and greater sensitivity than traditional culture-based techniques.

While multiplex molecular panels increase pathogen detection rates, they should not fully replace culture when antimicrobial susceptibility testing or public-health subtyping is required. Studies and systematic reviews note that molecular assays lack the isolate needed for susceptibility testing and epidemiologic serotyping; when Salmonella or other bacteria requiring culture-based confirmation are suspected, parallel culture or enrichment culture may be necessary to support treatment decisions and public health investigation.

Per IDSA guidance, routine testing is not recommended for noninflammatory diarrhea and acute gastroenteritis of limited duration. Testing is generally reserved for patients with features such as fever, bloody stools, severe abdominal pain, persistent symptoms (>7 days), nosocomial onset, or immunocompromise — not for brief, uncomplicated, afebrile, nonbloody diarrheal illness in otherwise healthy persons.

Broad, unrestricted use of highly multiplexed gastrointestinal panels for patients without guideline-based clinical indications is discouraged. Evidence reviews indicate panels detect more positives than conventional methods but the clinical significance of additional detections is uncertain, and increased false positives or incidental findings can lead to unnecessary interventions. Therefore, routine panel testing for patients lacking features that would affect management is of limited demonstrated utility.

Multiplex panel testing is not typically indicated for uncomplicated, self-limited, noninflammatory acute diarrhea (for example, afebrile, nonbloody illness of short duration) in immunocompetent patients. IDSA and the policy note that viral-targeted panels generally have limited clinical impact in these patients because viral gastroenteritis often resolves without treatment; viral panels may be considered primarily for immunocompromised individuals or infection-control needs.

Indications Covered by Policy

Evaluation of diarrhea >7 days; diarrhea with fever, bloody/mucoid stools, severe abdominal cramping/tenderness, or signs of sepsis; suspected enteric fever with recent travel; persistent diarrhea in immunocompromised or C. difficile–at-risk individuals when other stool studies are unrevealing.

Evaluation of the following clinical presentations supports use of multiplex panel testing when other criteria described in this policy are met:

Diarrhea for more than seven days.

Diarrhea with at least one of: fever; bloody or mucoid stools; severe abdominal cramping or tenderness; signs of sepsis.

Suspected enteric fever (typhoid or paratyphoid) in an individual with recent travel to an endemic region or exposure via food prepared by people with recent endemic exposure.

Persistent diarrhea in an immunocompromised individual or an individual at risk for C. difficile when other stool diagnostic studies have failed to yield a pathogen.

Prior authorization may be required for use of multiplex gastrointestinal NAAT panels. Unrestricted ordering of broad multiplex panels is discouraged.

Services Not Typically Covered

Routine testing of brief, self-limited noninflammatory diarrhea is considered not covered when results would not influence clinical management. Similarly, use of viral-only multiplex panels in otherwise immunocompetent individuals whose care will not change based on a viral result is not routinely indicated. Documented clinical features that would alter management (e.g., fever, bloody stools, severe or persistent symptoms, or immunocompromise) are expected to support medically necessary testing.

Panel-based testing for routine, uncomplicated acute gastroenteritis in patients without clinical indications is considered not covered due to limited demonstrated clinical utility. Systematic reviews find that multiplex panels detect more pathogens than conventional testing, but the added detections often have uncertain clinical significance and may increase false positives, leading to unnecessary treatment or follow-up.

Background and Evidence Summary

Gastrointestinal infections result from bacteria, viruses, protozoa, and parasites and can present across a spectrum from mild, self-limited diarrhea to severe, systemic illness. Many episodes of acute noninflammatory diarrhea resolve without testing or specific therapy. Molecular multiplex PCR panels have expanded diagnostic capability — they detect multiple pathogens rapidly and with higher sensitivity than some conventional methods — but their detection of nucleic acid does not distinguish viable from nonviable organisms and may identify mixed infections whose clinical relevance is unclear. Parasite testing should be considered when diarrhea persists beyond 7 days, and viral-only panels are principally useful in immunocompromised patients or for infection-control decisions.
