NAF and ductal cytology utility: Nipple aspirate fluid (NAF) cytology and ductal lavage have low sensitivity but can be highly specific in selected cohorts; adjunctive image analysis or molecular techniques may increase sensitivity but overall diagnostic performance and sample adequacy are limited.

Meta-analyses report poor sensitivity (~0.64) with high specificity (~0.97) and a high rate of inadequate samples (~39%); clinical utility for screening or diagnosis is therefore limited.

Major practice guidelines do not support routine use of ductal lavage (DL) or nipple smear cytology for breast cancer screening or diagnosis. The National Comprehensive Cancer Network (NCCN) states that DL is not recommended, and that nipple smear cytology is not routinely recommended for patients with nipple discharge who have no palpable abnormality (NCCN, 2025).

Retrospective and systematic review data show FDS can detect intraductal lesions and may have diagnostic value when other imaging fails to localize lesions.

Features that increase likelihood of malignancy and utility of FDS: Clinical features associated with higher malignancy risk and increased diagnostic yield for FDS include bloody (hemorrhagic) discharge, positive mammography, older or menopausal status, multiple or distal ductal lesions, irregular lesion morphology, hemorrhage of lesions, and abnormal ductal wall appearance.

Zhang et al. reported these features were associated with malignancy (p < 0.05) and that FDS detected intraductal lesions in patients with negative ultrasound/mammography.

Limitations when considering FDS: Although FDS can detect lesions (high sensitivity when any finding is considered positive), it has limited specificity for discriminating benign from malignant findings and study heterogeneity is substantial; use FDS selectively and interpret findings in the context of pathology and other imaging.
