Background, Evidence & Definitions

Ductal lavage (DL) is an invasive technique that removes ductal fluid by cannulating breast ducts via the nipple for cytologic analysis. Multiple clinical studies and series report that DL has low sensitivity, a substantial percentage of inconclusive or inadequate specimens, and that abnormal DL findings do not reliably predict subsequent development of breast cancer. There is no evidence DL reduces breast cancer mortality when added to standard screening; guidelines do not recommend its routine use.

Nipple aspirate fluid (NAF) cytology involves collecting nipple fluid (by aspiration or noninvasively) for cytologic or biomarker analysis. A systematic review/meta-analysis demonstrated poor sensitivity (~0.64) but high specificity (~0.97) and found a mean of 38.9% of samples inadequate for analysis — a major limitation to its clinical utility.

Fiberoptic ductoscopy (FDS) permits direct intraductal visualization using a thin endoscope and may be used with lavage or intraoperatively.
