Clinical scenarios for multiplex PCR panel (up to 5 targets) when performed as part of an evaluation that includes blood cultures
- Diarrhea >7 days: Diarrhea for more than 7 days
- Diarrhea with systemic or inflammatory features: Diarrhea with at least one of the following: Fever; Bloody or mucoid stools; Severe abdominal cramping or tenderness; Signs of sepsis
  List of 4 possible accompanying features; any one qualifies
- Suspected enteric fever with travel or exposure: Suspected enteric fever (typhoid, paratyphoid) in an individual with recent travel to an endemic region (e.g., South-Central Asia, Southeast Asia, Southern Africa) or who has consumed foods prepared by people with recent endemic exposure

Background & Evidence Summary

Gastrointestinal infections produce a wide range of clinical presentations and can be caused by bacteria, viruses, protozoa, and parasites; fecal testing is recommended for patients with moderate to severe, bloody, febrile, dysenteric, nosocomial, persistent diarrhea, or who are immunocompromised, whereas most noninflammatory or brief acute gastroenteritis is self-limited and does not require testing.

Molecular multiplex assays provide more rapid results and broader simultaneous detection of bacterial, viral, and protozoal pathogens compared with traditional culture, and they can improve diagnostic yield and shorten time-to-result.

Limitations of multiplex molecular panels include higher cost, the inability to provide isolates for antimicrobial susceptibility testing or subtyping (potentially necessitating culture for treatment guidance or public health surveillance), and detection of nucleic acid from nonviable organisms which may not indicate active infection — all of which require clinical correlation when interpreting positive results.
