Coverage and Medical Necessity Criteria

Covered: Multiplex PCR up to 5 targets with blood cultures

Covered when ANY of the following clinical indications are met (multiplex PCR panel up to 5 targets when performed with blood cultures):

A: Indications for 3-5 target panel: Diarrhea for more than 7 days; OR diarrhea with at least one of: fever, bloody or mucoid stools, severe abdominal cramping or tenderness, signs of sepsis; OR suspected enteric fever (typhoid or paratyphoid) in an individual with recent travel to an endemic region or who consumed foods prepared by someone with recent endemic exposure; OR immunosuppression (e.g., on immunosuppressive medications, post-transplant care, or AIDS).

These support use of multiplex PCR panels including up to 5 targets when performed as part of an evaluation that includes blood cultures.

Covered: Multiplex PCR 6-11 targets for persistent diarrhea

Covered when ALL of the following are met (multiplex PCR panel up to 11 targets for persistent diarrhea):

B: Indications for 6-11 target panel: Persistent diarrhea (diarrhea for more than 7 days) in an individual at risk for Clostridium difficile colitis or other immunosuppression and other stool diagnostic studies have failed to yield a pathogenic organism; AND presence of at least one of: fever, bloody or mucoid stools, severe abdominal cramping or tenderness, or signs of sepsis.

Supports use of multiplex PCR panels including up to 11 targets for evaluation of persistent diarrhea.

Coverage considerations and testing criteria

Evidence-based testing considerations and clinical utility observations from the literature (no explicit coverage algorithm present in this excerpt):

Use consistent with IDSA clinical criteria: Testing aligned with IDSA criteria (fever, abdominal pain, bloody stool, immunocompromised status) preserves clinical yield and can reduce unnecessary testing.

Clark et al. found that applying IDSA criteria could reduce testing by ~32% without reducing clinical yield (sensitivity ~97%).

When multiplex panel testing is clinically valuable: Consider multiplex testing when rapid pathogen identification would reasonably change antimicrobial therapy, infection control (isolation/precautions), or inpatient management.

IDSA and ACG recommendations emphasize targeted testing by clinical presentation and special considerations for travelers, Vibrio exposure, parasites in prolonged diarrhea, and C. difficile testing algorithms.

Test performance and limitations

Evidence considerations regarding test performance and clinical impact

Performance and impact: Multiplex panels detect more pathogens than conventional methods and, for some platforms (e.g., FilmArray), demonstrate high sensitivity (reported 94.5–100%) and specificity (97.1–100%) versus standard methods; however, additional positive detections may include false-positives or clinically uncertain findings, and culture-independent tests do not provide isolates for susceptibility or public-health subtyping and should not be considered tests-of-cure.

Freeman et al. systematic review and Buss et al. report increased detection and high analytic performance; ASM notes limited outcome-based evidence despite faster, accurate results.

Multiplex PCR panel testing for gastrointestinal pathogens when used for indications other than those specified as proven in this policy is considered unproven and not medically necessary. This includes any clinical uses not listed under the covered indications for 3–5 target or 6–11 target panels.

Multiplex PCR panels that include more than 11 targets are considered unproven and not medically necessary and may be denied. Providers should ensure panel selection and coding reflect the covered panel size (see CPT codes for 3–5 targets and 6–11 targets).

Multiplex molecular panels do not replace conventional culture when recovery of an isolate is required for antimicrobial susceptibility testing or public health subtyping. When culture or isolate-based testing is needed, positive molecular results may require follow-up culture to obtain organisms for susceptibility testing and epidemiologic investigation.

Culture-independent NAAT-based methods detect nucleic acid from both viable and nonviable organisms and therefore should not be used as a test of cure. The absence of culture limits the ability to obtain isolates for susceptibility testing or subtyping when those are clinically or public-health necessary.

Use of multiplex PCR panels outside the specified covered indications, and use of panels that exceed 11 targets, is considered unproven and not medically necessary. Routine ordering of broad, highly multiplexed panels when clinical criteria are not met is discouraged because additional detections may be of uncertain clinical significance and may not alter management.

Routine use of broad multiplex panels in low-yield scenarios—testing that does not meet guideline-based clinical criteria such as those recommended by the IDSA—is discouraged. Evidence shows that adherence to IDSA criteria preserves clinical yield and can substantially reduce unnecessary testing without reducing sensitivity.

Per IDSA guidance, no laboratory testing is typically indicated for noninflammatory diarrhea and acute gastroenteritis of limited duration. Stool testing is recommended primarily for patients with moderate to severe, bloody, febrile, dysenteric, nosocomial, or persistent diarrhea, or for immunocompromised patients.

Code selection must match the panel size being ordered. Use CPT codes that correspond to the number of targets in the multiplex PCR panel: 87505 for 3-5 targets, 87506 for 6-11 targets, and 87507 for 12-25 targets. Ordering a code that does not match the actual panel size may result in claim denial or payment adjustment.

Parallel culture recommendation

When PCR testing is used to detect Salmonella, perform enrichment culture in parallel. Enrichment culture improves sensitivity and supports antimicrobial susceptibility testing and public health subtyping; absence of culture confirmation may limit clinical and public health action.

Recommendation: run enrichment culture in parallel for Salmonella when PCR-based methods are used.

Clinical Background and Scope

Most acute, noninflammatory diarrheas are self-limited and do not require fecal testing. Fecal testing (culture or culture-independent methods) is recommended for patients with moderate-to-severe, bloody, febrile, nosocomial, or persistent diarrhea, and for immunocompromised individuals. Multiplex PCR panels may increase detection yield and shorten turnaround time compared with conventional methods, but they can identify organisms of uncertain clinical significance, do not routinely provide isolates for susceptibility or subtyping, and therefore should be used when results are likely to affect antimicrobial therapy, infection control, or inpatient management.
