Coverage Criteria for Multiplex Gastrointestinal Pathogen Panels

Medically Necessary Indications

Covered when ANY of the following clinical scenarios are met (with blood cultures as part of evaluation for the 3-5 target panel).

Initial/limited panel (3-5 targets): Multiplex PCR panel testing including up to 5 targets when performed as part of an evaluation that includes blood cultures for an individual with any one of: diarrhea > 7 days; or diarrhea with at least one of fever, bloody or mucoid stools, severe abdominal cramping/tenderness, signs of sepsis; or suspected enteric fever with recent travel to an endemic region or exposure via food preparer;up to 5 targets

Extended panel (6-11 targets) for persistent diarrhea: Multiplex PCR panel testing including up to 11 targets for evaluation of persistent diarrhea in an individual with any one of: at risk for Clostridium difficile colitis AND (diarrhea > 7 days OR diarrhea with fever OR bloody/mucoid stools OR severe abdominal cramping/tenderness OR signs of sepsis);6-11 targets

Also indicated for other conditions causing immunosuppression when other stool diagnostic studies have failed to yield a pathogenic organism.

Immunosuppressed individuals: Testing (panel up to 11 targets) is indicated for individuals with other causes of immunosuppression when prior stool diagnostic studies have not identified a pathogen; clinical judgment should guide selection of targets within this panel size.6-11 targets

IDSA testing criteria: Order molecular GI panels for patients meeting IDSA 2017 clinical criteria: fever, abdominal pain, bloody stool, or immunocompromised status; following these criteria has been shown to reduce testing volume without reducing clinical yield.

C. difficile testing: Use a two-step algorithm (a highly sensitive test followed by a highly specific test, e.g., NAAT plus toxin or antigen testing) to help distinguish colonization from active C. difficile infection.

Clinical interpretation considerations when panels are used

Interpretation cautions: NAAT-based multiplex panels detect nucleic acid and may not distinguish colonization from active infection; highly multiplexed assays can detect mixed infections and agents of uncertain clinical significance (eg, EAEC, EPEC, some viruses). Two-step algorithms are preferred for C. difficile because NAAT alone cannot distinguish colonization from infection. Culture-independent results should not be used as tests of cure and culture may be needed when susceptibility, subtyping, or public health reporting is required.

Guideline cautions and limitations documented in evidence

Multiplex PCR panel testing for gastrointestinal pathogens for indications other than those specifically listed as medically necessary in this policy is considered unproven and not medically necessary. Use of these panels outside the covered clinical scenarios lacks sufficient evidence of benefit and may be denied.

Multiplex gastrointestinal PCR panels that include more than 11 targets are considered unproven and not medically necessary. Panel sizes > 11 targets (CPT 87507) exceed the scope of supported clinical utility and may be subject to denial.

Culture-independent molecular assays do not replace traditional culture when culture is required for clinical or public health purposes. For pathogens where antimicrobial susceptibility testing, subtyping, or public health surveillance is needed (for example Salmonella, Shigella, Campylobacter), either culture should be performed in parallel or confirmatory culture/enrichment should be obtained because multiplex PCR may miss culture-positive cases or provide no isolate for susceptibility or typing.

Laboratory testing is generally not indicated for noninflammatory diarrhea and for acute gastroenteritis of limited duration. Routine use of multiplex panels for self-limited, noninflammatory diarrheal illness is not supported by guideline recommendations.

Use of multiplex PCR gastrointestinal pathogen panels for indications not enumerated in this policy is considered unproven and not covered. Testing outside the listed medical necessity criteria does not have sufficient evidence to demonstrate clinical benefit.

Broad, unrestricted use of multiplex gastrointestinal pathogen panels in low-yield populations (patients who do not meet the clinical criteria outlined in this policy) is of uncertain clinical value. Studies demonstrate increased detection of pathogens but also higher rates of findings of unclear significance and false positives, with limited impact on management when panels are applied without clinical selection.

Multiplex viral panels have limited clinical impact for viral gastroenteritis that typically resolves without specific therapy; routine viral panel testing in these cases is generally not indicated except in immunocompromised patients or other circumstances where viral identification would change management.

Consider using the IDSA 2017 criteria (e.g., fever, abdominal pain, bloody stool, or immunocompromised status) and other guideline-based indications (persistent or severe symptoms, travel-related enteric fever risk) when deciding to order testing.

Background

Gastrointestinal infections are caused by bacteria, viruses, protozoa, and parasites. Most noninflammatory and brief acute gastroenteritis is self-limited and does not require laboratory testing. Culture-independent multiplex PCR assays offer greater sensitivity and faster turnaround, and they are recommended for moderate-to-severe, bloody, febrile, nosocomial, persistent, or immunocompromised presentations; however, uncomplicated, short-duration diarrheal illness typically self-resolves and testing is not routinely necessary.
