Coverage Criteria for Saphnelo (anifrolumab-fnia)

Medical necessity criteria

Covered when ALL of the following are met:

Initial therapy: Diagnosis of moderate to severe systemic lupus erythematosus (SLE) without severe active central nervous system (CNS) lupus or severe active lupus nephritis; currently receiving at least one standard of care treatment for active SLE (e.g., antimalarials, corticosteroids, or immunosuppressants) that is not a biologic; history of failure, contraindication, or intolerance to Benlysta (intravenous or subcutaneous); patient is not receiving Saphnelo in combination with a biologic agent or Benlysta; Saphnelo is dosed according to FDA-labeled dosing for SLE; prescribed by or in consultation with a rheumatologist; initial authorization is for no more than 12 months.

Continuation of therapy: Patient has previously received Saphnelo injection or intravenous infusion; documentation of positive clinical response; patient remains without severe active CNS lupus or severe active lupus nephritis.

Saphnelo (anifrolumab-fnia) is considered unproven and not medically necessary when used in patients with severe active lupus nephritis. The policy reflects that the efficacy of Saphnelo has not been evaluated for this patient population and therefore its use is not recommended.

Saphnelo is also considered unproven and not medically necessary for patients with severe active central nervous system (CNS) lupus. The drug’s efficacy has not been evaluated in severe active CNS lupus, and use in this situation is not recommended.

Additionally, the policy states that Saphnelo should not be used in combination with other biologic therapies. Concomitant use with other biologics is not recommended and is considered an unsupported use.

Per policy limitations of use, Saphnelo’s safety and efficacy have not been established in severe active lupus nephritis or severe active CNS lupus, and therefore these indications are expressly not recommended.

The policy further clarifies that Saphnelo is not recommended for use with other biologic therapies; use in combination with other biologics is considered unproven and may result in non-coverage.

Background

Saphnelo (anifrolumab-fnia) is a human IgG1κ monoclonal antibody that acts as a type I interferon receptor antagonist, blocking type I interferon receptor signaling and reducing interferon-responsive gene expression and downstream inflammatory processes. It is FDA-indicated for adults with moderate to severe systemic lupus erythematosus (SLE) who are receiving standard therapy.
