Rheumatoid arthritis — Initial therapy

Covered when ALL of the following are met

RA initial criteria: Diagnosis of moderately to severely active rheumatoid arthritis (RA); one of the following: prior treatment with an FDA-approved targeted immunomodulator for RA OR history of failure or intolerance to a 3-month trial of one non-biologic DMARD (e.g., methotrexate, leflunomide, sulfasalazine, hydroxychloroquine) at maximally indicated doses unless contraindicated OR patient is currently on Orencia; Orencia is initiated and titrated according to FDA-labeled dosing for RA; prescribed by or in consultation with a rheumatologist; patient is not receiving Orencia in combination with a targeted immunomodulator (e.g., etanercept, certolizumab, golimumab, adalimumab, tofacitinib, baricitinib, upadacitinib).

Initial authorization is for no more than 12 months.

Psoriatic arthritis — Initial therapy

Covered when ALL of the following are met

PsA initial criteria: Diagnosis of active psoriatic arthritis (PsA); one of the following: prior treatment with an FDA-approved targeted immunomodulator for PsA OR history of failure to a 3-month trial of methotrexate at maximally indicated dose unless contraindicated OR patient is currently on Orencia; Orencia is initiated and titrated according to FDA-labeled dosing for PsA; prescribed by or in consultation with a rheumatologist or dermatologist; patient is not receiving Orencia in combination with a targeted immunomodulator (e.g., certolizumab, adalimumab, golimumab, ustekinumab, risankizumab, guselkumab, secukinumab, ixekizumab, tofacitinib, baricitinib, upadacitinib, apremilast).

Initial authorization is for no more than 12 months.

Chronic graft-versus-host disease (cGVHD)

Covered when ALL of the following are met

cGVHD criteria: Diagnosis of steroid-refractory chronic graft-versus-host disease (cGVHD); for initial therapy, one of the following: patient is intolerant to systemic corticosteroid therapy OR patient is receiving Orencia in combination with systemic corticosteroids; documentation of positive clinical response required for continuation; Orencia is dosed per the clinical protocol or FDA labeling as applicable.

Initial authorization is for no more than 12 months.

Acute graft-versus-host disease (aGVHD)

Covered when ALL of the following are met

aGVHD criteria: Patient is at least 2 years old AND undergoing hematopoietic stem cell transplantation (HSCT) from a matched donor OR undergoing HSCT from a 1 allele-mismatched unrelated donor; patient is receiving Orencia in combination with a calcineurin inhibitor and in combination with methotrexate as specified in the clinical studies; dosing schedule consistent with studies (Day -1, and Days 5, 14, and 28); authorization is limited to no more than 4 doses.

Immune checkpoint inhibitor‑related severe cardiotoxicity
