SLE — Initial and Continuation Therapy

Covered when ALL of the following are met:

Diagnosis of active systemic lupus erythematosus, without severe active central nervous system lupus.

Standard therapy: For initial therapy: currently receiving at least one standard of care non-biologic treatment for active SLE (e.g., antimalarials, corticosteroids, or immunosuppressants). For continuation: currently receiving at least one standard of care non-biologic treatment.

Standard therapy: For initial therapy: currently receiving at least one standard of care non-biologic treatment (e.g., corticosteroids plus mycophenolate or cyclophosphamide/azathioprine induction/maintenance as used in trials). For continuation: currently receiving at least one standard non-biologic therapy.

Dosing: Benlysta is initiated and titrated (initial) or dosed (continuation) according to US Food and Drug Administration labeled dosing (studied regimen: 10 mg/kg IV on Days 0, 14, 28, then every 28 days).
