Coverage Criteria for Benlysta (belimumab)

Systemic Lupus Erythematosus

Benlysta is proven and medically necessary for treatment of systemic lupus erythematosus when ALL of the following are met:

Initial therapy for SLE: Diagnosis of active systemic lupus erythematosus, without severe active central nervous system lupus; and currently receiving at least one standard-of-care non-biologic therapy for active SLE (e.g., antimalarials, corticosteroids, or immunosuppressants); and patient is not receiving Benlysta in combination with Lupkynis (voclosporin) or Saphnelo (anifrolumab-fnia); and Benlysta is initiated and titrated according to US Food and Drug Administration labeled dosing for SLE; and initial authorization is for no more than 12 months.

Continuation therapy for SLE: Patient has previously received Benlysta injection for intravenous infusion; and documentation of positive clinical response; and currently receiving at least one standard-of-care non-biologic therapy for active SLE (e.g., antimalarials, corticosteroids, or immunosuppressants); and patient is not receiving Benlysta in combination with Lupkynis (voclosporin) or Saphnelo (anifrolumab-fnia); and Benlysta is dosed according to US Food and Drug Administration labeled dosing for SLE; and authorization is for no more than 12 months.

Active Lupus Nephritis

Benlysta is proven and medically necessary for treatment of active lupus nephritis when ALL of the following are met:

Initial therapy for active lupus nephritis: Diagnosis of active lupus nephritis, without severe active central nervous system lupus; and currently receiving at least one standard-of-care non-biologic therapy for active lupus nephritis (e.g., antimalarials, corticosteroids, or immunosuppressants); and patient is not receiving Benlysta in combination with Lupkynis (voclosporin) or Saphnelo (anifrolumab-fnia); and Benlysta is initiated and titrated according to US Food and Drug Administration labeled dosing; and initial authorization is for no more than 12 months.

Per indication language

Use of belimumab for certain indications is considered unproven and not medically necessary. Specifically, belimumab is not supported by established evidence for antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis, rheumatoid arthritis, severe active central nervous system (CNS) lupus, Sjögren's syndrome, use in combination with other biologic agents, or Waldenström macroglobulinemia.

The efficacy of Benlysta has not been evaluated in patients with severe active CNS lupus, and its use in this population is not recommended. In addition, belimumab has not been adequately studied in combination with other biologic agents; such combination therapy is therefore considered unproven and may not be covered.

Belimumab is regarded as unproven and not medically necessary for multiple indications where sufficient evidence of clinical benefit is lacking. These include ANCA-associated vasculitis, rheumatoid arthritis, severe active CNS lupus, Sjögren's syndrome, use in combination with other biologics, and Waldenström macroglobulinemia.

Excluded or unsupported indications include, but are not limited to: ANCA-associated vasculitis, Waldenström macroglobulinemia, Sjögren's syndrome, and rheumatoid arthritis. These uses are considered investigational or unproven due to a lack of large, controlled trials demonstrating improved health outcomes.

Background

Belimumab is a recombinant human monoclonal antibody that binds soluble B-lymphocyte stimulator (BLyS/BAFF). By neutralizing BLyS/BAFF, belimumab reduces B-cell survival and differentiation into immunoglobulin-producing plasma cells, which underlies its mechanism of action as an adjunctive therapy for active systemic lupus erythematosus and active lupus nephritis.
