EPOCH / DA-EPOCH / R-EPOCH (Lymphoma): EPOCH / DA-EPOCH / R-EPOCH regimen with drugs: Etoposide 50 mg/m2/day continuous infusion days 1-4; Prednisone; Vincristine IV days 1-4; Cyclophosphamide 750 mg/m2 IV on day 5; Doxorubicin 10 mg/m2/day continuous infusion days 1-4; with or without Rituximab
Factor: coordination of multiple infusions/multiple drugs over 96 hours
ESHAP / R-ESHAP (Lymphoma): ESHAP / R-ESHAP regimen with drugs: Etoposide 40 mg/m2/day continuous infusion days 1-4; Methylprednisolone; Cytarabine (high-dose Ara-C) 2 g/m2; Cisplatin 25 mg/m2 continuous infusion days 1-4; with or without Rituximab

Background & Evidence

Most chemotherapy can be administered safely in a physician's office or via home healthcare, but certain regimens, drugs, cancer types, and clinical factors necessitate an observation stay or inpatient hospitalization due to the need for close monitoring, multi-day continuous infusions, or management of potential toxicities. Examples include the need for monitoring during continuous multi‑day infusions (e.g., EPOCH/DA‑EPOCH/R‑EPOCH, ESHAP/ R‑ESHAP, Hyper‑CVAD), continuous cardiac or intensive monitoring (e.g., Interleukin‑2), close serum monitoring and leucovorin rescue for high‑dose methotrexate (> 500 mg/m2), management of high‑dose ifosfamide (> 1 g/m2/day) with urine alkalinization and electrolyte replacement, and intra‑arterial procedures such as Transcatheter Arterial Chemoembolization (TACE) or other intra‑arterial infusions.
