FDA guidance on loss of response and PRCA

FDA safety advisory considerations:

FDA-directed actions: Any patient who develops sudden loss of response with severe anemia and low reticulocyte count should be evaluated for neutralizing antibodies; if suspected, withhold ESAs and contact the manufacturer for binding and neutralizing antibody assays; prescribing information contains warnings regarding PRCA for darbepoetin alfa, epoetin alfa, and MPGe-epoetin beta.

Additional exclusions: ESAs are considered unproven and are not indicated for patients receiving hormonal agents, biologic products, or radiotherapy alone unless they are also receiving concomitant myelosuppressive chemotherapy. ESAs are unproven for patients undergoing cardiac or vascular surgery, for patients who require immediate correction of anemia as a substitute for red blood cell transfusion, and for patients scheduled to donate autologous blood. Requests for ESA therapy in these settings are expected to be denied unless a specific, contractually permitted exception is documented.

Safety and hemoglobin targets: document counseling on ESA risks and ensure individualized targets. The work group suggests avoiding maintenance of hemoglobin above 11.5 g/dL in adult CKD patients and explicitly recommends not intentionally increasing hemoglobin above 13 g/dL. Providers should record the clinical rationale when targeting higher concentrations and obtain informed consent when patients accept known risks.

Limitation for cancer patients not receiving myelosuppressive chemotherapy: ESAs are not indicated for patients with cancer who are not receiving therapy or who are receiving non-myelosuppressive therapy. Coverage for oncology-related anemia is restricted to patients receiving myelosuppressive chemotherapy (and only per the criteria for non-curative settings or other specified exceptions). Requests for ESA use outside these chemotherapy contexts should be declined unless an evidence-based exception is documented.

FDA approvals are informational only: FDA indications and prescribing information are provided for reference, but FDA approval alone does not guarantee coverage. Coverage decisions must still align with plan, state, federal, or contractual requirements and with the specific clinical criteria in this policy.

Use during curative-intent chemotherapy is unproven/not medically necessary: ESA therapy for patients undergoing curative chemotherapy is considered unproven and generally not medically necessary except when a patient explicitly refuses blood transfusion. In such rare circumstances, documentation must justify the exception and demonstrate that standard transfusion management is not acceptable to the patient.

Risks associated with higher hemoglobin targets: randomized trials and meta-analyses show ESAs reduce transfusion requirements but are associated with increased thromboembolic events and, in some analyses, increased mortality. For example, pooled cancer trials reported reduced transfusion risk (RR 0.65) but increased mortality during the active study period (HR 1.17) and higher thromboembolic risk (RR 1.52). These safety signals support conservative hemoglobin targets and careful risk–benefit documentation.

Explicit recommendation on upper hemoglobin limits: do not intentionally increase hemoglobin above 13 g/dL. KDIGO–informed guidance further suggests avoiding maintenance of hemoglobin above 11.5 g/dL in adults with CKD unless an individualized judgment—documented and accepted by the patient—supports a higher target.
