Coverage Criteria — Observation vs Inpatient

Inpatient medical necessity criteria

Inpatient stay is medically necessary when ANY of the following regimen- or condition-based criteria are met:

Regimens requiring inpatient monitoring: EPOCH/DA-EPOCH/R-EPOCH for lymphoma when coordination of multiple infusions or multiple drugs over 96 hours is required; ESHAP/R-ESHAP for lymphoma with monitoring for CNS toxicity related to cytarabine; Interleukin-2 infusion (600,000 IU/kg IV every 8 hours for up to 14 consecutive doses) for melanoma or renal cell carcinoma when continuous cardiac monitoring, close monitoring of electrolytes/renal/hepatic function and possible vasopressor support are needed; high-dose ifosfamide (infusion > 1 g/m2/day) for sarcoma requiring close monitoring of serum electrolytes and urine pH with alkalinization; high-dose methotrexate (>500 mg/m2) with leucovorin rescue for lymphoma or sarcoma requiring methotrexate level monitoring; Hyper-CVAD cycles requiring coordination of multiple infusions over 96 hours, bladder irrigation, and close methotrexate level monitoring.multi-day continuous infusions, high-dose definitions provided in policy
