Inpatient stay medically necessary

Covered as medically necessary when any of the following inpatient‑requiring regimens or circumstances are present in adults (age 9):

Specific conditions requiring inpatient stay: Specific diagnoses or procedures that require inpatient hospitalization, including acute leukemia, intra-arterial chemotherapy infusion (including Transcatheter Arterial Chemoembolization [TACE]), and prophylaxis of tumor lysis syndrome in specified high‑risk lymphoma cases.

Observation or outpatient appropriate with clinical selection

Observation stay may be appropriate for certain chemotherapy‑related clinical conditions or complications; outpatient administration may be acceptable when monitoring needs and patient selection allow:

Observation-appropriate situations and evidence: Selected regimens and patients can be managed safely in an observation unit or outpatient setting when appropriate selection and monitoring are in place (examples from the literature include outpatient administration of DA-EPOCH in selected patients, outpatient ifosfamide regimens, and outpatient high‑dose cytarabine in selected populations). Clinical studies report feasible and, in some cohorts, safe outpatient delivery with admission to inpatient units reserved for complications such as febrile neutropenia or neurotoxicity; patient selection and monitoring requirements are emphasized.

This policy applies only to adults. It does not apply to individuals under 18 years of age. Coverage determinations and the criteria in this document are intended for use with adult patients receiving chemotherapy who may require observation or inpatient hospital care.
