Medically necessary indications for observation or inpatient hospitalization

Inpatient hospitalization is medically necessary for regimens or clinical conditions that meet any of the following (examples provided):

Inpatient-required chemotherapy regimens: EPOCH, DA-EPOCH, or R-EPOCH when coordination of multiple continuous infusions (etoposide and doxorubicin) and multiple drugs over approximately 96 hours are required; ESHAP or R-ESHAP when coordination of multiple continuous infusions over approximately 96 hours is required and monitoring for CNS toxicity from high-dose cytarabine is needed; Hyper-CVAD cycles requiring coordination of multiple infusions/drugs over ~96 hours, bladder irrigation with cyclophosphamide, and close methotrexate level monitoring; Interleukin-2 infusion when continuous cardiac monitoring, close laboratory monitoring, potential vasopressor support, and proximity to intensive care are required; high-dose ifosfamide (>1 g/m2/day) when close monitoring of serum electrolytes and urine pH with electrolyte replacement and urine alkalinization is required; high-dose methotrexate (>500 mg/m2) with leucovorin rescue when serial methotrexate level monitoring and specified leucovorin dosing are required.

Clinical conditions necessitating inpatient stay: Acute leukemia (clinical context may necessitate inpatient management for complications); intra-arterial infusion of chemotherapy (including TACE) requiring facility-based administration; prophylaxis or treatment of tumor lysis syndrome in high-grade lymphoma with large tumor burden when inpatient monitoring is indicated.

Conditions that may require observation stay: Congestive heart failure or chronic renal failure requiring high-volume fluid infusions; known prior hypersensitivity reactions to infusion therapy; TACE or intra-arterial chemotherapy infusions that may be managed in observation with monitoring rather than full inpatient admission.
