Inpatient medical necessity criteria

Inpatient hospitalization is medically necessary for regimens or clinical conditions that meet the following groups (examples provided in policy):

Regimens requiring inpatient monitoring: EPOCH, DA-EPOCH, or R-EPOCH when coordination of multiple infusions or multiple drugs over 96 hours is needed; ESHAP or R-ESHAP when coordination of multiple infusions or multiple drugs over 96 hours and monitoring for CNS toxicity with cytarabine is needed; Hyper-CVAD (cycles 1,3,5,7 and alternating cycles with high-dose methotrexate/cytarabine) when coordination of multiple infusions over 96 hours, bladder irrigation with cyclophosphamide, or close methotrexate level monitoring is required; High-dose methotrexate (>500 mg/m2) with leucovorin rescue requiring serum methotrexate monitoring; High-dose ifosfamide (ifosfamide infusion >1 g/m2/day) requiring close monitoring of serum electrolytes and urine pH, electrolyte replacement, and urine alkalinization; Interleukin-2 infusion (600,000 IU/kg IV q8h up to 14 doses) when continuous cardiac monitoring, close monitoring of electrolytes/renal/hepatic function, vasopressor support availability, and proximity to ICU are required.

Any requests for an extension of an inpatient stay beyond the recommended day(s) must be clinically reviewed. The provider should submit clinical rationale and relevant documentation to support the extension request for review prior to or at the time of the requested extension.

Include progress notes, updated vital signs and nursing assessments, medication administration record, laboratory and imaging results relevant to the continued need for inpatient care.
Document the specific clinical issues necessitating the extension, anticipated duration, and planned interventions or monitoring.
Ensure the facility follows a written protocol for administration of the chemotherapy drug(s).
