Services identified as not medically necessary are excluded and may be subject to denial. Examples include administration of a medication by injection when standard medical practice indicates that the oral route is an effective, accepted, or preferred method; administration that exceeds the frequency or duration indicated by standard medical practice; and off‑label medication use that does not meet the criteria in the Benefit Interpretation Policy titled Medications and Off‑Label Drugs.

Maximum member cost share (California statute)Notwithstanding any deductible, total copayments and coinsurance for an individual prescription of up to a 30‑day supply of a prescribed orally administered anticancer medication shall not exceed $250.
