Covered when ALL of the following apply

Covered chemotherapy services: Medically necessary chemotherapy treatment when used according to FDA approved indications or as part of a cancer treatment regimen.

Includes injectable medications (including intravenous infusion therapy), appointments to monitor chemotherapy, inpatient or outpatient chemotherapy treatment, and laboratory services provided on the same day as chemotherapy.

Oral chemotherapy coverage conditions: Oral anticancer medications and other oral chemotherapy-related medications are covered when the member has a supplemental outpatient prescription benefit or when federal/state mandated regulations require coverage terms (e.g., California HSC §1367.656 / Insurance Code §10123.206).

Refer to the member's Evidence of Coverage (EOC)/Schedule of Benefits (SOB) and applicable federal/state mandated regulations for specific benefit eligibility and any cost‑sharing limits.

Off‑label use of a medication is covered only if it meets the criteria in the Benefit Interpretation Policy titled 'Medications and Off‑Label Drugs'.

Examples of non‑medically necessary chemotherapy services include medication given by injection when standard medical practice indicates an effective and accepted oral or preferred method exists, and administration of medications that exceed the frequency and duration indicated by standard medical practice. Additional non‑covered items include off‑label medication use that does not meet the Medications and Off‑Label Drugs policy, oral medications when the member lacks a supplemental outpatient prescription benefit or a mandated benefit, and transportation and lodging costs.

Medication administered by injection is considered Not Covered when standard medical practice indicates an effective and accepted or preferred oral (by mouth) alternative. Exceptions to this exclusion are not listed here; coverage for injections instead of oral therapy would need to meet medical necessity and any applicable benefit criteria or fall under an allowed exception in the member's Evidence of Coverage/Schedule of Benefits or related off‑label drug policy.
