Chelation therapy is not indicated for a broad list of chronic, non-overload conditions. The policy identifies numerous non‑overload uses that lack high‑quality evidence of benefit, including but not limited to cardiovascular disease, atherosclerosis, diabetes, cancer, Alzheimer's disease, Parkinson's disease, rheumatoid arthritis, chronic renal insufficiency, chronic fatigue syndrome, and many other listed conditions. These indications were evaluated in systematic reviews and health technology assessments which found studies to be small, heterogeneous, and methodologically limited, preventing conclusions of clinical efficacy.

When chelation is proposed for these non‑overload conditions, providers should be aware that the literature does not demonstrate meaningful patient‑centered benefit and that safety concerns and methodological issues (heterogeneous populations, nonstandard regimens, limited follow‑up, and lack of adequate controls) have been repeatedly noted. Requests for chelation for these non‑overload diagnoses are therefore considered unproven and are subject to denial.

Chelation therapy is limited to the treatment of documented metal intoxication diagnosed using validated laboratory testing in appropriate biological samples. Multiple professional societies, including the American College of Medical Toxicology (ACMT) and the American Academy of Family Physicians (AAFP), state that chelation should not be used for conditions other than confirmed metal poisoning or clinically significant overload.

Requests for chelation to treat any condition other than a documented metal intoxication (for example, attempting to treat chronic disease states without objective, validated evidence of metal toxicity) are considered investigational or excluded and may be denied.

Use of chelation to treat autism spectrum disorder (ASD) is specifically not supported. A Cochrane review found no evidence that pharmaceutical chelation improves ASD symptoms, and trial reports note serious adverse events (including hypocalcemia, renal impairment, and death) associated with chelation in this population. Given the lack of demonstrated benefit and potential for harm, chelation for ASD is considered not medically necessary.

Chelation to improve symptoms or reduce risk in stable ischemic heart disease (IHD) or other cardiovascular conditions is likewise not supported. Guideline panels (including ACP/ACC/AHA and the Canadian Cardiovascular Society) recommend against using chelation to improve angina, exercise tolerance, or to reduce cardiovascular risk, and the FDA has warned that chelating agents are not approved to treat autism or related conditions.

Overall, chelation therapy for non‑overload uses is considered not medically necessary. Systematic reviews and health technology assessments conclude that available studies for these indications are generally weak, inconsistent, or show no clinical benefit. The policy therefore does not support routine use of chelation outside of validated indications for metal poisoning or documented overload.

Specific examples include attempts to use chelation for chronic progressive diseases, removal of purported ‘toxins’ without demonstrated overload, or to treat symptoms of neurodegenerative and chronic systemic diseases. Because evidence does not demonstrate efficacy and safety concerns exist, such uses are subject to denial.

EDTA/disodium EDTA chelation has been extensively studied for cardiovascular indications, most notably in the TACT trial series. The TACT pivotal trial randomized participants to receive 40 infusions of an EDTA‑based chelation solution versus placebo and included a factorial randomization with high‑dose oral vitamins; follow‑up extended to five years. Systematic reviews that included TACT report no consistent, high‑certainty evidence of reduced mortality or clear improvement in major cardiovascular outcomes.

Subgroup analyses (for example, participants with diabetes) have suggested exploratory signals of benefit in some analyses, but these findings are considered hypothesis‑generating and insufficient to support routine clinical use. Guideline committees conclude that EDTA chelation should not be used to improve symptoms or reduce cardiovascular risk in stable ischemic heart disease.

Reiterating the policy position, chelation for autism spectrum disorder is not supported by clinical trial evidence and is associated with potentially serious adverse events; therefore it is considered not medically necessary.

Similarly, chelation to improve angina, exercise tolerance, or to reduce cardiovascular risk in ischemic heart disease is not supported by guideline recommendations and is considered not medically necessary for those indications.
