NK1 RA indication group: 1) BOTH: prevention of chemotherapy-induced nausea and vomiting due to High Emetic Risk parenteral anticancer agents AND in combination with a 5HT3 RA; OR 2) ALL: prevention of chemotherapy-induced nausea and vomiting due to Moderate Emetic Risk parenteral anticancer agents AND in combination with a 5HT3 RA AND presence of at least one risk factor for anticancer-agent induced nausea/vomiting (younger age <55 years; female sex; previous history of chemotherapy-induced nausea or vomiting; little or no previous alcohol use; history of motion sickness or morning sickness during pregnancy; high anxiety).

Guideline-concordant prophylaxis and breakthrough management

Covered when antiemetic selection follows guideline-recommended prophylaxis according to regimen emetogenicity and for breakthrough management per guideline escalation:

Emetogenicity-based prophylaxis: Select prophylactic antiemetic regimen based on emetogenicity: for high emetic risk use multi-drug regimens (NK1 receptor antagonist + 5-HT3 receptor antagonist + dexamethasone ± olanzapine per ASCO/NCCN); for moderate emetic risk use 2–3 drug combinations as specified (include an NK1 antagonist when carboplatin AUC ≥ 4); for low/minimal risk use single agent or as-needed therapy per NCCN/ASCO guidance.

Breakthrough CINV management: If breakthrough nausea/vomiting occurs despite optimal prophylaxis, escalate to a higher-level regimen for subsequent cycles; if olanzapine was not given, consider adding olanzapine; if olanzapine was already used, consider adding a drug from a different class (for example an NK1 antagonist, benzodiazepine, dopamine antagonist, dronabinol, or nabilone) in addition to continuing the standard regimen.

Medicare Coverage Triggers

Medicare coverage may differ for certain oral antiemetics. Oral aprepitant (Emend capsules) may be covered by Medicare only when criteria of the National Coverage Determination (NCD) 110.18 are met. Local Coverage Determinations (LCDs)/Local Coverage Articles (LCAs) may apply for other oral agents. Absence of meeting applicable NCD/LCD/LCA criteria or plan rules may result in denial for Medicare members. Step Therapy Requirement

Step therapy applies for non-preferred NK1 RA, 5HT3 RA, and NK1 RA/5HT3 RA combination products. Approval for a non-preferred product generally requires a trial of an adequate dose and duration of a preferred NK1 RA or 5HT3 RA with documented minimal clinical response, intolerance, or contraindication, plus physician attestation as specified in the policy.

Therapy Escalation for Breakthrough CINV

If breakthrough chemotherapy-induced nausea and vomiting (CINV) occurs despite optimal prophylaxis, escalate therapy for subsequent chemotherapy cycles by advancing to a higher-level antiemetic regimen per guideline recommendations (for example, adding or substituting agents such as an NK1 RA, olanzapine, or a different drug class). Document the evaluation of emetic risk, current regimen, and rationale for escalation in the medical record.

Consider adding olanzapine for patients who did not receive it prophylactically.
If olanzapine was already used, consider a drug from a different class (e.g., NK1 RA, benzodiazepine, dopamine antagonist, dronabinol, nabilone) in addition to the standard regimen.
Adjustments should be documented and applied to subsequent cycles.
