Covered when ALL of the following are met:

Core requirements: The affected individual displays signs or symptoms of an undiagnosed or unexplained disorder with a suspected genetic cause; test results are intended to directly impact the individual's medical management; the clinical presentation does not fit a Well‑Delineated Genetic Syndrome or disorder for which a specific test or Targeted Panel is available (if a specific syndrome is suspected, single‑gene or targeted panel testing should be performed first); and the test is ordered by a medical geneticist, neonatologist, neurologist, immunologist, or developmental pediatrician.

Covered when ALL of the following are met:

Prenatal WES criteria: Chromosomal microarray analysis (CMA) and/or karyotyping has been performed and was uninformative; WES is ordered by or in consultation with a medical geneticist or a maternal‑fetal medicine specialist (perinatologist); specimen is obtained from amniotic fluid and/or chorionic villi, or DNA is extracted from fetal blood or tissue; and the fetus has either multiple congenital anomalies affecting ≥2 organ systems, fetal hydrops of unknown etiology, or a congenital anomaly of a single organ system with a family history suggestive of a genetic etiology.

ES reanalysis can yield additional diagnoses (pooled reanalysis yields reported up to ~43% in selected series), underscoring the value of reanalysis workflows.

Reanalysis considerations:

Periodic reanalysis of prior ES/WGS data: Reanalysis or resequencing of previously nondiagnostic ES/WGS data can yield additional diagnoses and should be considered periodically, particularly when the phenotype evolves or new gene‑disease associations are reported; studies support reanalysis strategies and recommend intervals such as every 12–24 months with many groups recommending a minimum interval of ≥18 months before routine reanalysis.

inv-10: Guideline-recommended clinical contexts for WES/WGS

Professional society recommendations indicate WES/WGS is covered/appropriate in the following clinical contexts when performed by accredited laboratories and with appropriate clinical and genetic counseling:

ACMG pediatric/congenital anomalies recommendation: WES/WGS is recommended as a first‑ or second‑tier test for individuals with one or more congenital anomalies with onset prior to one year of age or for individuals with developmental delay/intellectual disability with onset prior to 18 years of age.

ACMG practice guideline supports WES/WGS in these groups (chunk 72).

Prenatal society positions (ISPD/ACOG/ACMG): Consider prenatal WES/WGS when fetal ultrasound shows a major single anomaly or multiple organ system anomalies or when CMA/karyotype is nondiagnostic and a genetics expert deems the phenotype suggestive of a single‑gene disorder; routine prenatal WES/WGS is not recommended outside trials or specified circumstances.

ISPD and ACOG statements provide conditional support and caution (chunks 78, 74).

Epilepsy society guidance (ILAE/NICE/NSGC): WES/WGS (including CNV analysis) is recommended as first‑line testing for many individuals with epilepsy—particularly severe childhood‑onset epilepsies, epilepsy with intellectual disability/autism, progressive epilepsies, and when clinical features suggest a genetic epilepsy syndrome; genetic counseling and specialist input are recommended.

WES and WGS are not medically necessary when used outside the policy’s defined indications. Examples of uses considered unproven and not medically necessary include evaluation of fetal demise, prenatal testing via cell-free fetal DNA, preimplantation genetic testing in embryos, and screening or evaluation of individuals who do not meet the clinical criteria in this policy. The policy also specifies that rapid or ultra-rapid sequencing (rWES, rWGS, urWGS) is not medically necessary in outpatient settings, and that whole transcriptome sequencing, epigenetic signature analysis, and optical genome mapping (OGM) are considered unproven and not medically necessary for any indication at this time.

Optical genome mapping (OGM) and epigenetic signature (episignature) analysis are currently under investigation and are not established as routine clinical diagnostics. OGM uses imaging of ultra-long DNA molecules to detect structural variants but evidence of clinical utility is limited to proof-of-principle and small studies. Epigenetic signature analysis (DNA methylation episignatures) shows diagnostic potential in selected, unresolved cases but remains investigational pending further validation and broader clinical utility data.

Systematic reviews and meta-analyses show that prenatal ES/WGS in unselected, phenotypically normal fetuses after normal CMA/karyotype has a very low pooled incremental diagnostic yield (pooled estimate ~1.6%), and moderate or severe disease caused by de novo variants is even lower (~0.7%). By contrast, selected prenatal cohorts with structural anomalies demonstrate substantially higher incremental yields. These data support restricting routine prenatal ES/WGS in phenotypically normal pregnancies and using sequencing primarily when there is a clinical indication.

Rapid sequencing approaches (rWES, rWGS, urWGS) have demonstrated meaningful diagnostic yields in critically ill, hospitalized infants, but current evidence does not support their use in outpatient settings. Whole transcriptome sequencing and optical genome mapping remain investigational for routine clinical use and lack sufficient evidence to be recommended outside research or specific second‑tier contexts.

WES/WGS is not indicated for screening healthy children. Professional societies (ASHG, ACOG) advise that routine prenatal WES/WGS is not recommended outside clinical trials or narrowly defined clinical circumstances; when a well‑delineated genetic syndrome is suspected, targeted single‑gene testing or a focused panel is preferred before proceeding to genome‑scale assays.

Coverage is subject to the member’s federal, state, or contractual benefit terms, which may impose additional exclusions or requirements. Providers should verify plan‑specific prior authorization and benefit rules before ordering, as this policy is informational and does not supersede contract provisions.

Technologies and test approaches considered unproven or investigational in this policy include rapid WES/rWGS/urWGS in outpatient settings, whole transcriptome sequencing (RNA‑seq), optical genome mapping (OGM), and epigenetic signature (episignature) analysis for routine diagnostic use. These may be considered in research or selected second‑tier cases but are not standard covered indications.

Hayes systematic reviews identified insufficient evidence to conclude that WES or WGS improves clinical decision‑making or outcomes for adults with suspected neuromuscular or movement disorders, and highlighted the limited quality and quantity of available studies. In some contexts GS as a secondary test also delayed diagnosis, underscoring the need for appropriate test sequencing and higher‑quality prospective studies.

Performing ES/WGS solely at parental request in the absence of fetal anomalies or other clinical indications is not supported by current evidence because the incremental diagnostic yield in phenotypically normal fetuses after normal standard testing is very low (~1.6% pooled).

Use of rapid or ultra‑rapid sequencing outside critically ill inpatient populations is not supported by current evidence and therefore is considered investigational for outpatient use. Other investigational uses not supported include routine first‑line use of whole transcriptome sequencing, OGM, and episignature analysis for general constitutional diagnostics.

Eligibility and Preconditions for Testing

A documented family history of a similar clinical diagnosis in a first‑ or second‑degree relative is listed among qualifying features and can contribute to meeting eligibility for WES/WGS. Family history and pedigrees should be documented in the medical record to support clinical necessity and interpretation.

Clinical and testing documentation requirements include detailed prenatal phenotyping (ultrasound/MRI findings), documentation of prior karyotype and/or CMA results, and clear family history. For postnatal cases, provide clinical findings, prior genetic testing results, and family structure to assist laboratories with variant interpretation.
