The following are excluded from coverage: genetic testing performed solely to determine fetal sex, testing performed for individuals who are not UnitedHealthcare members, and genetic testing or counseling requested for non-medical reasons such as court-ordered tests, work-related tests, or paternity testing. Coverage decisions are subject to the member's Evidence of Coverage (EOC) and Schedule of Benefits (SOB); the EOC/SOB governs if there is any discrepancy between this policy and a member’s plan documents.

Testing, treatment, counseling, or screening to determine carrier status in newborns, children, or adolescents is considered not medically necessary when there would not be an immediate medical benefit or when the results would not be used to initiate medical interventions or treatment during the newborn, childhood, or adolescent period. As with all services, coverage is contingent on medical necessity and the member’s EOC/SOB.
