Unproven / Not Medically Necessary: Ductal lavage; breast ductal fluid aspiration and cytology (including nipple aspirate fluid cytology); and fiberoptic ductoscopy, with or without ductal lavage, are considered unproven and not medically necessary for breast cancer screening, breast cancer diagnosis, or as alternative tools to guide surgery due to insufficient evidence of efficacy, low sensitivity, high rates of inconclusive/insufficient samples, limited diagnostic accuracy, and lack of demonstrated impact on breast cancer mortality.

Supported by practice guideline recommendation (NCCN: DL not recommended) and limited/low-quality evidence; further longitudinal studies are needed to establish outcome benefit.

Breast ductal lavage (DL), breast ductal fluid aspiration and cytology (nipple aspirate fluid, NAF), and fiberoptic ductoscopy (FDS) are considered unproven and not medically necessary for breast cancer screening, breast cancer diagnosis, or as alternative tools to guide surgery. The policy lists these interventions explicitly as lacking sufficient evidence of efficacy and therefore not appropriate for routine clinical use in these indications.

There is no high-quality evidence that ductoscopy improves breast cancer detection rates, leads to earlier stage diagnosis, or reduces breast cancer mortality. Published studies are largely preliminary cross‑sectional and heterogeneous, and systematic reviews have concluded that additional outcome-focused research is required to establish clinical benefit.

Based on available evidence, the following procedures are considered not medically necessary / unproven for breast cancer screening, diagnosis, or as alternatives to standard surgical guidance: breast ductal lavage, breast ductal fluid aspiration and cytology (NAF), and fiberoptic ductoscopy (with or without ductal lavage). Limitations cited include low sensitivity, high rates of inadequate or inconclusive samples, and lack of demonstrated impact on clinical outcomes.

Ductoscopy can detect many intraductal abnormalities but does not reliably distinguish malignant from benign findings. Meta-analytic data show wide heterogeneity in sensitivity and specificity depending on definition and interpretation, and study-level variation undermines consistent diagnostic discrimination. These limitations mean the technique lacks demonstrated outcome benefit across clinical settings.

Imaging Preference Before Fiberoptic Ductoscopy (FDS)

High-frequency ultrasound (HFUS) is the preferred initial imaging modality for pathologic nipple discharge (PND) when it can establish the cause, location, and extent of a lesion. Reserve fiberoptic ductoscopy (FDS) for cases where HFUS is insufficient to localize or characterize the lesion; combining HFUS and FDS may improve diagnostic accuracy when HFUS alone is nondiagnostic.

Use HFUS first for evaluation of PND and duct ectasia when feasible
If HFUS does not reveal the lesion or is nondiagnostic, consider FDS in conjunction with HFUS
Document why HFUS was insufficient when seeking authorization for FDS
