Site-of-Service Medical Necessity Criteria

Medically necessary clinical and access reasons

A planned surgical procedure performed in a hospital outpatient department is considered medically necessary when ANY one of the following clinical conditions or access limitations is met:

Clinical conditions: Advanced liver disease (MELD Score > 8); Advance surgical planning determines individual requires overnight recovery and care following a surgical procedure; Anticipated need for transfusion; Bleeding disorder requiring replacement factor or blood products or special infusion products to correct a coagulation defect; Cardiac arrhythmia (symptomatic arrhythmia despite medication); Chronic obstructive pulmonary disease (COPD) with FEV1 < 50%; Coronary artery disease (CAD)/peripheral vascular disease (PVD) with ongoing ischemia or recently placed drug eluting stent (within 1 year); Developmental stage or cognitive status warranting hospital outpatient department use; End stage renal disease on dialysis with hyperkalemia above reference range; Recent cerebrovascular accident (CVA) or transient ischemic attack (TIA) (within 3 months); Recent myocardial infarction (MI) (within 3 months); Individuals with drug eluting stents placed within 1 year or bare metal stents/plain angioplasty within 90 days unless antiplatelet plan agreed by surgeon, cardiologist, and anesthesia; Poorly controlled asthma (FEV1 < 80% despite medical management); Pregnancy; Prolonged surgery (> 3 hours); Resistant (poorly controlled) hypertension; Severe valvular heart disease; Moderate to severe obstructive sleep apnea (OSA); Uncompensated chronic heart failure (NYHA class III or IV); Uncontrolled diabetes with recurrent DKA or severe hypoglycemia; Age under 18 unless required by federal, state, or contractual requirementssee individual items

Each listed condition alone can justify hospital outpatient department placement when present.

Access limitations

A planned surgical procedure is considered medically necessary in a hospital outpatient department if there is an inability to access an ASC for any one of the following reasons:

Access limitations: An ASC's specific guideline regarding the individual's health conditions or weight that would preclude management of the individual within an ASC setting; No geographically accessible ASC has the necessary equipment for the procedure (examples include, but are not limited to: fluoroscopy, laser, ocular equipment, operating microscope, nonstandard scopes such as duodenoscope or ureteroscope); No geographically accessible ASC where the individual's physician has privilegesN/A

Excludes surgeon-preferred or proprietary instruments, instrument sets, or hardware sets as a reason for hospital outpatient department placement.

This policy explicitly excludes non-clinical reasons for selecting a hospital outpatient department. Surgeon-preferred or proprietary instruments, instrument sets, or hardware sets are not acceptable justifications for performing a planned procedure in a hospital outpatient department instead of an ASC. Only the access limitation that an ASC lacks required equipment (as described elsewhere) is considered — preference for specific proprietary tools is not a covered reason to require the hospital setting.

When none of the listed clinical conditions or the access limitations are present for the individual, the hospital outpatient department location may not be considered medically necessary for the planned surgical procedure. If the outpatient hospital department is not considered medically necessary under the member's benefit plan, that location will not be covered.

Denial Risk

Noncovered location if hospital outpatient setting is not medically necessary

If the outpatient hospital department is not considered medically necessary under the member’s benefit plan, that location will not be covered by the member’s plan.

Site-of-service medical necessity is determined in accordance with the terms of the member’s benefit plan.
Coverage for the hospital outpatient location is denied when the location is not medically necessary under the plan.

Clinical Rationale

Some patients undergoing planned surgical procedures require the hospital outpatient department because of higher clinical risk or complexity. Examples include individuals with advanced liver disease (MELD score > 8), significant cardiopulmonary disease such as COPD with FEV1 < 50% or symptomatic arrhythmia despite medication, conditions that make overnight recovery necessary, anticipated need for transfusion, or bleeding disorders requiring replacement products. In these situations the hospital outpatient setting is a medically necessary site of care to ensure access to appropriate monitoring, blood products, and higher-acuity resources.
