Observation services are not medically necessary when provided solely for convenience of the hospital, physicians, the individual, or the individual’s family. Observation level care is also not medically necessary when used while awaiting placement or transport to another health care facility. Specific presenting problems that are addressed under the policy’s Coverage Limitations and Exclusions (for example, pneumonia, seizures, TIA, poisoning/toxic ingestions, renal colic, syncope, vaginal bleeding that is non‑obstetrical, urinary tract infection, and weakness) are listed in the policy and may affect coverage determinations.
