Coverage Criteria and Clinical Indications

Medical necessity criteria by procedure type

Covered when ALL of the following are met for the specific procedure category

General requirements for breast surgery (mastectomy, breast reduction, augmentation): Written clinical assessment from at least one Qualified Healthcare Professional experienced in treating Gender Dysphoria documenting: persistent, well-documented Gender Dysphoria; capacity to make a fully informed decision and to consent for treatment; minimum age requirements (must be at least 18 years of age for breast augmentation; must be at least 18 years of age for mastectomy or breast reduction though individuals within one calendar year of turning 18 can be considered on a case-by-case basis); favorable psychosocial-behavioral evaluation to identify risk factors or potential postoperative challenges; and for breast augmentation, continued Gender Dysphoria following completion of 12 months of continuous hormone therapy prior to the procedure.

Voice modification/masculinization surgery: Written clinical assessment from at least one Qualified Healthcare Professional experienced in treating Gender Dysphoria documenting: persistent, well-documented Gender Dysphoria; capacity to consent; minimum age of 18 years; favorable psychosocial-behavioral evaluation; completion of 6 months continuous hormone therapy prior to surgery for voice masculinization when required; and documentation of presurgical voice lessons and/or therapy.

Genital surgery: Written clinical assessments from at least two Qualified Healthcare Professionals experienced in treating Gender Dysphoria, who have independently assessed the individual, documenting: persistent, well-documented Gender Dysphoria; capacity to make a fully informed decision and to consent for treatment; minimum age of 18 years; favorable psychosocial-behavioral evaluation; completion of at least 12 months of successful continuous full-time real-life involvement in the identified gender; completion of 12 months of continuous hormone therapy appropriate for the experienced gender unless medically contraindicated or not indicated; and a treatment plan that includes ongoing follow-up and care by a Qualified Healthcare Professional experienced in treating Gender Dysphoria.

Covered procedures list: Bilateral mastectomy or breast reduction; breast augmentation with implants or fat transfer; clitoroplasty; hysterectomy; labiaplasty; laser or electrolysis hair removal in advance of genital reconstruction when prescribed by a physician for treatment of Gender Dysphoria; metoidioplasty; orchiectomy; penectomy; penile prosthesis; phalloplasty; salpingo-oophorectomy; scrotoplasty; testicular prostheses; thyroid cartilage reduction (tracheal shave); urethroplasty (female and male); vaginectomy; vaginoplasty; voice lessons and/or voice therapy; voice modification surgery (e.g., laryngoplasty, glottoplasty, shortening of the vocal cords); vulvectomy; and related procedures listed in the Coverage Rationale.

Cosmetic exclusions: Abdominoplasty; blepharoplasty; body contouring (e.g., fat transfer, lipoplasty, panniculectomy); brow lift; calf implants; cheek, chin, and nose implants; face/forehead lift and/or neck tightening; facial bone remodeling for facial feminization; hair transplantation; injection of fillers or neurotoxins; laser or electrolysis hair removal not related to genital reconstruction; lip augmentation or reduction; liposuction; mastopexy; pectoral implants for chest masculinization; rhinoplasty; skin resurfacing (e.g., dermabrasion, chemical peels, laser); and similar ancillary procedures when performed as part of gender-affirming care.

Covered services when plan and legal requirements are met:

Plan & legal coverage: Benefit coverage for health services is determined by federal, state, or contractual requirements; when a plan covers treatment for Gender Dysphoria, coverage includes psychotherapy, gender-affirming hormone therapy, puberty-suppressing medications, laboratory monitoring to monitor safety of hormone therapy, and certain surgical treatments listed in the Coverage Rationale. Certain plans may not cover all listed surgical treatments — refer to plan-level/contractual requirements for specifics.

Genital Surgery — recommended prerequisites

Surgical genital gender-affirming procedures are addressed with prerequisites and timing recommendations according to specialty guidance.

Genital gender-affirming surgery prerequisites (per Endocrine Society): Patient is at or above the age of majority (suggested ≥18 years); the mental health practitioner and the clinician responsible for endocrine transition therapy agree that surgery is medically necessary and would benefit the patient's overall health and/or well-being; completion of at least 1 year of consistent and compliant hormone treatment unless hormone therapy is not desired or is medically contraindicated; the physician responsible for endocrine treatment medically clears the individual for surgery and collaborates with the surgeon regarding hormone use during and after surgery; and consideration that timing recommendations are based on low to very low-quality evidence.

Recommendations include strong and ungraded practice statements from Endocrine Society guidance.

Ancillary Surgery/Procedures — evidence and individualized consideration

Ancillary procedures (facial feminization, body-contouring, hair removal, voice/communication) — evidence and coverage considerations

Ancillary procedures evidence summary: Available studies are generally poor- to very poor-quality, often retrospective and small; while many report high patient satisfaction and potential improvements in quality of life, data are limited for validated patient-reported outcome measures, long-term follow-up, and standardized functional outcomes. Coverage decisions should consider individualized clinical benefit and documentation of functional or psychosocial need; Hayes evolving evidence reviews and consensus panels note potential benefit but limited evidence quality.

Individualized documentation of clinical necessity and anticipated benefit is recommended when evaluating coverage for these procedures.

Adolescent Gender-Affirming Surgery

Procedures in adolescents

Adolescents and GAS: Evidence for genital gender-affirming surgery in adolescents is extremely limited or lacking; chest (mastectomy) in transgender males may provide psychological benefits but long-term data are limited. Surgical interventions in adolescents are occasionally pursued on a case-by-case basis with multidisciplinary input, careful consideration of irreversible effects and fertility implications, and alignment with guidance from pediatric and professional societies.

AAP and Hayes emphasize case-by-case multidisciplinary decision-making and the limited evidence base for adolescent GAS.

The policy identifies a set of ancillary cosmetic procedures that are considered not medically necessary when performed as part of gender‑affirming treatment. Examples include abdominoplasty, blepharoplasty, body contouring (fat transfer, lipoplasty, panniculectomy), brow lift, calf implants, cheek/chin/nose implants, face/forehead lift, facial bone remodeling for facial feminization, hair transplantation, injection of fillers or neurotoxins, laser or electrolysis hair removal not related to genital reconstruction, lip augmentation or reduction, liposuction, mastopexy, pectoral implants for chest masculinization, rhinoplasty, and skin resurfacing.

This Medical Policy does not apply to individuals with ambiguous genitalia or disorders of sexual development; see the Benefit Considerations and related policies for those clinical scenarios.

Certain services are excluded from coverage. Examples include treatment received outside the United States and reproductive services such as sperm preservation, cryopreservation of fertilized embryos, oocyte preservation, surrogate parenting, donor eggs, donor sperm, and use of a host uterus. Additionally, cosmetic procedures that fall under separate Cosmetic and Reconstructive Procedures policy or are specifically excluded by federal, state, or contractual requirements are not covered. Coverage also does not apply to members who do not meet the indications listed in the Coverage Rationale.

FDA information is provided for reference only. Gender transformation surgeries are procedures and not subject to FDA regulation. Devices, drugs, biologics, or tests used in these procedures may be regulated by the FDA, but FDA approval or clearance of such products is informational and is not itself a basis for coverage decisions.

When performed as part of gender‑affirming surgery, the policy explicitly lists a number of ancillary cosmetic procedures that are considered separate from medically necessary gender‑affirming treatments and are therefore treated as cosmetic. Examples provided include abdominoplasty, blepharoplasty, body contouring (fat transfer, lipoplasty, panniculectomy), facial bone remodeling for facial feminization, hair transplantation, injection of fillers or neurotoxins, laser/electrolysis hair removal not related to genital reconstruction, lip augmentation/reduction, liposuction, mastopexy, pectoral implants, rhinoplasty, and skin resurfacing.

Coverage under this policy does not apply to members who do not meet the indications specified in the Coverage Rationale section. Providers should confirm that all required medical necessity criteria and supporting documentation are present before submitting requests for authorization.

Available evidence for many individual gender‑affirming surgical procedures is limited. Existing systematic reviews and studies are frequently retrospective, heterogeneous, and lack validated outcome measures and long‑term follow‑up, which limits confidence in estimating the magnitude of benefit and harm for many procedures.

Background and Evidence Summary

Gender dysphoria is defined by DSM‑5‑TR criteria as a marked incongruence between an individual's experienced or expressed gender and the gender assigned at birth, present for at least 6 months, and associated with clinically significant distress or impairment. Assessment and treatment decisions should be based on documented, persistent gender dysphoria and an evaluation of capacity to consent and psychosocial functioning by appropriately qualified clinicians.

Systematic reviews of gender‑affirming surgical procedures report substantial limitations in the literature, including lack of controlled studies, limited prospective data, high loss to follow‑up, and absence of validated outcome measures. Because of these limitations, the policy states that the magnitude of benefit and harm for individual surgical procedures cannot be accurately estimated for many procedures.
