Virtual upper GI endoscopy cannot obtain tissue samples. Because it is an imaging procedure and does not permit biopsy, it is not a substitute when tissue sampling is required. When a lesion is identified by virtual imaging, conventional upper GI endoscopy is necessary for biopsy and further management. Conventional endoscopy remains the standard approach when direct visualization with the option for tissue diagnosis or therapeutic intervention is indicated.

Virtual upper GI endoscopy (also called virtual gastroscopy) uses three-dimensional CT or MRI data sets with image reconstruction techniques — for example, multiplanar reformats, volume renderings, and simulated endoscopic views — to visualize the internal surfaces of the esophagus, stomach and proximal small bowel without inserting an endoscope. The procedure is noninvasive and typically does not require sedation; captured images can be magnified and viewed from different angles.
