Per oral endoscopic myotomy (POEM) is unproven and not medically necessary when proposed for indications other than Achalasia or Diffuse Esophageal Spasm. The policy explicitly states that POEM lacks sufficient evidence of efficacy for other uses such as treatment of Zenker's diverticulum (Z-POEM), and long-term safety and effectiveness data are inadequate to support coverage for these non‑approved indications.

The evidence is insufficient to support the safety and effectiveness of radiofrequency energy (e.g., the Stretta system) for gastroesophageal conditions beyond the indications evaluated in controlled studies. Endoscopic plication, suturing, and polymer injection or implantation techniques for GERD lack high-quality, long-term evidence. Studies are generally small, nonrandomized, and show progressive loss of benefit over time in some series; objective physiologic control (manometry or pH) is inconsistently improved, limiting support for coverage when durable efficacy is required.

Procedures that have not demonstrated adequate long-term objective reflux control or that lack comparative evidence to established treatments (for example some TIF studies) may be excluded from coverage when long-term physiologic control is required. Network analyses and systematic reviews have found inferior physiologic outcomes for some endoscopic approaches compared with laparoscopic fundoplication, and durability of effect diminishes in longer follow-up.

The use of EndoFLIP (functional lumen imaging probe) as a standalone diagnostic replacement for esophageal manometry, or as the definitive guide for procedural decision-making, is not supported by sufficient high-quality comparative evidence. Reviews and technology assessments report inconclusive results and call for larger, controlled studies before EndoFLIP can be relied upon as an independent diagnostic or procedural guidance tool.

The policy excludes POEM as unproven and not medically necessary for indications outside of Achalasia and Diffuse Esophageal Spasm. Zenker's diverticulum (Z-POEM) is specifically cited as an example where the current evidence base is insufficient to establish safety and efficacy.

EndoFLIP use for routine diagnostic work-up, as procedural guidance, or for post-procedural outcome assessment is not supported by comparative evidence. Technology assessments and systematic reviews highlight limited, inconclusive data and recommend further larger controlled studies before routine clinical adoption for these purposes.
