For all other indications, including procedures such as treatment of Zenker's diverticula, POEM is considered unproven and not medically necessary because available evidence is insufficient to demonstrate efficacy.

The Stretta radiofrequency system has an overall low-quality evidence base with mixed results across observational studies and assessments. A 2020 randomized, double-blind, sham-controlled multicenter trial (Zerbib et al.) found no significant difference between Stretta and sham at 24 and 48 weeks for days without heartburn, other digestive symptoms, PPI/antacid use, or patients not taking PPIs. Systematic assessments conclude Stretta may have potential benefit but remains unproven compared with surgical fundoplication and requires further robust, long-term randomized data to establish effectiveness.

Multiple endoscopic plication, suturing, injection and other minimally invasive GERD techniques lack high-quality, long-term evidence. Studies are commonly limited by small sample sizes, nonrandomized designs, short follow-up, inconsistent objective testing, and post hoc analyses. Available series report symptomatic benefit that often diminishes over time (progressive loss of effect reported up to 60 months), and these limitations mean such procedures do not have sufficient high-quality evidence to confirm durable safety and effectiveness.

Several earlier endoscopic anti-reflux devices and injectable materials (for example, Enteryx, Plexiglas, Durasphere and other injectables) and some prior-generation implantable devices lack adequate long-term safety and effectiveness data. Systematic reviews and small series highlight limited follow-up, heterogeneity of outcomes, and inconsistent comparative evidence; consequently these devices remain unsupported by sufficient long-term evidence.

EndoFLIP (Functional Lumen Imaging Probe) is an adjunctive impedance planimetry system with investigational applications across esophageal and gastric disorders. Its use as the sole diagnostic determinant for therapeutic decisions is limited because systematic reviews and technology assessments report insufficient, low-quality evidence and note that precise EGJ distensibility index (DI) thresholds associated with treatment success are not established. Standardized DI protocols and validated cutoffs are lacking, restricting EndoFLIP's ability to independently guide management.

This medical policy must be applied together with any applicable federal, state, or contractual benefit plan requirements. If those requirements differ from the policy, the federal, state, or contractual terms will govern coverage decisions. Providers should verify applicable benefit plan rules and prior authorization requirements before proceeding.

Summary stance: the policy considers the following procedures unproven and not medically necessary based on insufficient high-quality evidence—G-POEM for gastroparesis; routine use of EndoFLIP as a diagnostic test for achalasia; endoluminal therapy with GERDx; and select endoscopic/implant techniques for GERD (including many prior-generation injectables and some plication/suturing methods). Additionally, Stretta lacks sham-controlled trial support for benefit over sham and is therefore regarded as unproven for routine treatment of refractory GERD without compelling additional evidence.

The 2020 randomized, double-blind, sham-controlled trial of the Stretta system (Zerbib et al.) randomized 62 patients with moderate to severe PPI-refractory GERD to radiofrequency treatment or sham. At planned follow-up visits through 48 weeks, there were no significant differences between groups in heartburn-free days, other digestive symptom–free days, or PPI/antacid use. The trial authors concluded Stretta did not demonstrate efficacy over sham in this population.

Endoscopic plication and suturing approaches (and related endoluminal techniques) show variable short-term symptom improvements but are characterized by limited or conflicting long-term data. Systematic reviews and case series report progressive loss of symptomatic benefit in some cohorts and a paucity of randomized, controlled long-term effectiveness and safety data, limiting routine coverage consideration.

Several earlier endoscopic anti-reflux devices and injectables have not demonstrated consistent long-term outcomes and are supported primarily by small series and heterogeneous studies. Because long-term safety and effectiveness remain uncertain for many of these devices, their routine use is not supported by the current evidence base.
