Coverage Criteria for Multiplex Gastrointestinal Pathogen PCR Panels

Medically necessary indications

Multiplex PCR panel testing of gastrointestinal pathogens is considered proven and medically necessary when ONE of the following indications is met:

General/3-5 target panels or smaller multiplex testing: Diarrhea for more than 7 days; OR multiplex PCR panel testing (including up to five targets) when performed as part of an evaluation that includes blood cultures for an individual with any one of: diarrhea with bloody or mucoid stools; fever; severe abdominal cramping or tenderness; signs of sepsis.

Supports single or smaller multiplex panels as part of evaluation including blood cultures where indicated.

Suspected enteric fever (eg, typhoid or paratyphoid) in an individual with a history of recent travel to an endemic region (eg, South Central Asia, South East Asia, Southern Africa) or who has consumed foods prepared by people with recent endemic exposure.

Include blood cultures as part of evaluation when enteric fever or bacteremia is suspected.

At risk for C. difficile — up to 11-target panels: For individuals at risk for Clostridium difficile colitis: multiplex PCR panel testing including up to 11 targets for evaluation of persistent diarrhea in an individual with any one of: diarrhea for more than 7 days; fever; diarrhea with bloody or mucoid stools; signs of sepsis; severe abdominal cramping or tenderness; OR patient is on immunosuppressive medications (post‑transplant or for autoimmune disease); OR has acquired immune deficiency syndrome (AIDS); OR has another condition causing immunosuppression and other stool diagnostic studies have failed to yield a pathogenic organism.
