Coverage Criteria for Ambulance Services

Emergency Air Ambulance - Medically Necessary

Covered when ALL of the following are met

Air ambulance medical necessity: 1) The member's medical condition requires immediate transportation that cannot be provided by ground ambulance and a delay in transportation time may endanger the member's life or seriously endanger the member's health (for example: ground transport times are excessive (i.e., 30-60 minutes or longer); weather or traffic make ground ambulance transportation impractical, impossible, or overly time consuming; or the pickup point is inaccessible by ground ambulance).

Ground ambulance medical necessity: 1) The member's medical condition requires immediate transportation to the nearest acute hospital that can provide services appropriate to the covered person's illness or injury (including to the nearest neonatal special care unit for newborns or to a hospital that provides a required higher level of care not available at the original hospital).

Covered ambulance services

General coverage: Ambulance services are covered when it is the only safe way to transport a member to the nearest hospital or other care facility due to the nature or severity of the injury or illness; services may be provided via ground, air, or water and the mode of transportation is determined by appropriate authorities (e.g., first responders, emergency room physician).

Non-emergency interfacility coverage conditions: Non-emergency ambulance transportation between facilities is covered when any one of the following is met: from an out-of-network hospital to the closest network hospital when Covered Health Care Services are required; to the closest network hospital that provides the required Covered Health Care Services not available at the original hospital; or from a Short-Term Acute Care Facility to the closest network LTAC, inpatient rehabilitation facility, or other network Sub-Acute Facility where the required Covered Health Care Services can be delivered.

Benefit level depends on origin and member setting (inpatient vs sub-acute).

Non-Emergency Ambulance - Covered Interfacility Transport

Covered when ANY of the following interfacility conditions are met

Interfacility transfer conditions: From an out-of-network hospital to the closest network hospital when Covered Health Care Services are required.

Applies to licensed ambulance services (ground or air) as determined by UnitedHealthcare.

Interfacility transfer conditions: To the closest network hospital that provides the required Covered Health Care Services that were not available at the original hospital.

Applies to licensed ambulance services (ground or air) as determined by UnitedHealthcare.

Interfacility transfer conditions: From a Short-Term Acute Care Facility to the closest network Long-Term Acute Care Facility (LTAC), network inpatient rehabilitation facility, or other network Sub-Acute Facility where the required Covered Health Care Services can be delivered.

Appropriate Use Criteria for Air Medical Transport

Air medical transport is appropriate when at least one of the following is met:

Primary patient-centered goals: 1) Initiation or continuation of advanced or specialty care that is not otherwise available locally from hospital or ground EMS (GEMS) resources.

Based on NAEMSP/ACEP/AMPA joint statement.

Primary patient-centered goals: 2) Expedited delivery of a patient to definitive care for time-sensitive intervention.

Primary patient-centered goals: 3) Extraction, evacuation, and/or rescue from environments that are difficult to access due to geography, weather, remote location, distance, or other limiting factors.

Ground EMS preference:

Emergency Air Ambulance services are considered Medically Necessary only when the coverage criteria for air transport are fully met. If the criteria are not satisfied, Emergency Air Ambulance services are not considered Medically Necessary for all other indications. This includes any air transport that does not meet the listed requirements for necessity, destination, or on-scene/transport clinical needs.

Emergency ground ambulance transportation is covered only when all emergency ground criteria are present (immediate need to nearest appropriate hospital, delay would endanger life or health, and ALS or BLS required during transport). Emergency ground ambulance transportation is not considered Medically Necessary for all other indications, except that emergency ground ambulance treatment rendered at the scene without subsequent transport may be medically necessary when performed by ambulance personnel.

Any ambulance transport (air or ground) that does not meet the specified emergency criteria or the specified non-emergency interfacility criteria is considered not medically necessary. Non-emergency interfacility transport is covered only when one of the interfacility conditions is met (e.g., transfer from an out-of-network hospital to the closest network hospital that can provide required Covered Health Care Services). Transports failing to meet these defined conditions are ineligible for coverage.

Excluded from coverage are ambulance services provided by entities that are not properly licensed, air ambulance transports that do not meet the documented air-ambulance criteria, and any non-ambulance transportation modes. Examples of non-covered non-ambulance modes include commercial or private airline or helicopter, police car transport, medi-van or wheelchair van, taxi, bus, or rideshare services such as Lyft or Uber. Additional exclusions include transports to home/residential/custodial facilities, transports for member convenience or preference, and transports that violate notification criteria.

Air medical response should be aligned with the primary patient-centered goals (access to advanced/specialty care not available locally; expedited delivery for time-sensitive interventions; or extraction/evacuation from difficult-to-access environments). Continuation of an air response should be canceled when it would place the air or ground crews at undue risk or would not align with those goals.

Ambulance transportation is not covered when other modes of transportation would not endanger the member's health. Routine, preference, or convenience transports (for example, hospital-to-home, home-to-physician-office, or scheduled home-to-hospital services) are not medically necessary. Triage and clinical documentation should support why ambulance transport was required; absent documentation showing that other modes would endanger the member, claims may be denied.
