Advanced intermittent pneumatic compression devices (APCDs) intended for treatment of lymphedema of the head, face, or neck are designated in this policy as unproven and not medically necessary. The policy notes insufficient peer-reviewed evidence to establish efficacy, clinical value, or safety for APCDs in these anatomic locations and indicates further controlled research is needed to define their role.

Pneumatic compression devices that use a high-pressure, rapid inflation/deflation cycle for treatment of peripheral arterial disease (PAD) are considered unproven and not medically necessary. The document states the evidence is inconsistent and insufficient to determine safety and efficacy of high-pressure devices for arterial insufficiency.

Trials evaluating IPC commonly excluded patients with specific contraindications. Follow these contraindications when selecting mechanical prophylaxis.

Routine use of intermittent pneumatic compression (IPC) alone as a substitute for appropriate pharmacologic thromboprophylaxis is not consistently supported by the evidence. Systematic reviews and guideline statements note that most evidence does not support mechanical prophylaxis alone and that IPC should not routinely replace pharmacologic prophylaxis when anticoagulation is appropriate and tolerated.
