Covered when ALL of the following are met:

Gait trainer - functional ambulation: The individual is 18 years of age or younger; AND the individual has the potential for Functional Ambulation; AND the individual uses the gait trainer when documentation shows assistive devices have not been effective.

inv-02: Gait trainers for therapeutic ambulation

Covered when ALL of the following are met:

Gait trainer - therapeutic ambulation: The individual is 18 years of age or younger; AND the individual is capable of utilizing and tolerating the equipment safely; AND the individual requires moderate to maximum support for ambulation (i.e., handheld ambulation assist devices are not feasible); AND the individual has an acquired injury (e.g., spinal cord or traumatic brain injury) or a chronic physical limitation that affects the ability to ambulate (e.g., cerebral palsy, neuromuscular disease, or spina bifida); AND the individual has a physician-directed written treatment plan (including frequency and duration).

inv-03: Supported standing systems

Covered when ALL of the following are met:

Standing systems general criteria: There is a goal of prevention of one or more specified medical complications (decubitus ulcer needing off-loading not achievable by other means; osteoporosis where improvement/stabilization of bone mineral density cannot be achieved with other treatments or activities; high potential for progressive contracture formation; compromised bowel/bladder with incomplete emptying or constipation refractory to other medical treatment; pulmonary complications with recurrent infections and poor clearance despite other treatment; worsening hip subluxation/dislocation not responsive to alternate treatments); AND the individual is unable to accomplish the above goals with current medical device/equipment or alternate medical treatment; AND the individual has been evaluated in physical therapy with a trial using the standing device and has shown compliance, tolerance, and demonstrated potential for clinical benefit, as determined by the evaluator; AND there is a written plan of care.

Use of supported standing systems is excluded when the stated prevention or therapeutic goals can be accomplished with the member’s current medical device/equipment or with alternate medical treatment. The policy requires documentation that the individual is unable to accomplish the stated goals with current/alternate treatment before a standing system will be considered medically necessary.
