Coverage and Medical Necessity Criteria

Standing Frame (medical necessity)

Standing frames are covered when ALL of the following are met:

Standing frame medical necessity criteria: The beneficiary is at high risk for lower extremity contractures that cannot be improved with other interventions (e.g., stretching, medications, serial casting, splinting, modalities); can tolerate a standing or upright position on the foot and ankle; is non-ambulatory or unable to stand due to neuromuscular or congenital disorders or acquired skeletal abnormalities; has tried more cost-effective alternatives and still requires a stander; does not have a walker or gait trainer and it is not anticipated they will require one; has demonstrated improved mobility, function, or physiologic symptoms or has maintained status with the use of the requested stander and is able to follow a home standing program with the use of the requested stander; and will use the equipment for personal use only (not for school).

Gait Trainer for Functional Ambulation

Gait trainers for functional ambulation are covered when ALL of the following are met:

Gait trainer - functional ambulation: The individual is 18 years of age or younger; has the potential for Functional Ambulation; and uses the gait trainer when documentation shows assistive devices have not been effective.

Gait Trainer for Therapeutic Ambulation

Gait trainers for therapeutic ambulation are covered when ALL of the following are met:

Gait trainer - therapeutic ambulation: The individual is 18 years of age or younger; is capable of utilizing and tolerating the equipment safely; requires moderate to maximum support for ambulation (handheld ambulation assist devices are not feasible); has an acquired injury or a chronic physical limitation that affects ambulation (e.g., spinal cord or traumatic brain injury, cerebral palsy, neuromuscular disease, spina bifida); and has a physician-directed written treatment plan including frequency and duration.

From Pediatric Gait Trainers - Therapeutic Ambulation

A stander will not be purchased when the beneficiary already has a gait trainer or another ambulatory device, or when the equipment is intended for use at school rather than for personal/home use. The standing frame criteria require that equipment be used for personal use only and note that the beneficiary must not have a walker or gait trainer and it is not anticipated they will require one.

Use of standing frames is considered not medically necessary when there is no expected improvement in mobility or maintenance of function. The policy also indicates non-coverage when anticipated functional benefits can be achieved through less-costly alternatives.

Required documentation must include a physician‑directed written treatment plan, records of prior conservative or less‑costly treatments tried, and evidence of therapeutic benefit and tolerance (improved mobility/function or maintained status with a home program).

Non-Covered Items and Situations

The policy excludes coverage for mobile (dynamic) standers and active standers, and for beneficiaries with certain contraindicating conditions. Specifically, standing frames are not covered for beneficiaries with complete paralysis of the lower extremities, or for those with conditions such as syncope, orthostatic hypotension, postural tachycardia syndrome, osteogenesis imperfecta, osteoporosis and other brittle bone diseases, and hip subluxation. Additionally, beneficiaries with hip and knee flexion contractures of more than 20 degrees are listed in the non-coverage instructions.

The policy lists specific device types and circumstances that are not covered. Mobile (dynamic) standers and active standers are explicitly excluded. Non-coverage also applies for beneficiaries with the contraindicating conditions noted above (e.g., syncope, orthostatic hypotension, brittle bone diseases, hip subluxation) and for situations where less-costly alternatives would achieve the anticipated benefits. In addition, the policy notes that certain HCPCS-coded items identified with an asterisk may not be covered by the State of Louisiana Medicaid Fee Schedule; for example, E0642 (mobile/dynamic stander) is indicated as not on the State of Louisiana Medicaid Fee Schedule and therefore may not be covered.

Background

Gait trainers and supported standing systems are assistive devices for children with mobility limitations. Gait trainers provide trunk and pelvic support and unweighting to assist stepping and can be configured with posterior, upright, or anterior support depending on the child’s needs. Supported standing systems secure users in an upright position and may be static (single or multi-position) or dynamic/mobile; the policy distinguishes these device types because mobile/active standers are excluded from coverage. Evidence summarized in the policy indicates mechanically assisted gait training may yield small improvements in walking speed and gross motor function for children with cerebral palsy, though overall evidence is limited and often descriptive. The policy applies to pediatric beneficiaries (age ≤ 18) and requires documentation of potential for functional or therapeutic ambulation, ability to tolerate and use the device safely, prior conservative treatments, and a physician-directed written treatment plan.
